According to Rust, CHCs serve as patient-centered medical homes to uninsured patients by employing a team-based, physician-led model of care. According to the January 23, 2009, MMWR, five cases of Hib disease in children younger than five years were reported to the Minnesota Department of Health in 2008; one of the children died from the disease.
