CAA Title V requires that all major stationary sources of air pollutants apply for and be issued operating permits. When Title V was added to the CAA, Congress expected that all of the Title V permits would be issued within seven years (by November 1997); however, as of December 2001, only 70% of sources have been issued Title V permits. EPA's Office of the Inspector General (OIG) initiated an evaluation in response to concerns expressed by EPA Region 5 management regarding the progress state and local air pollution control agencies are making in issuing Title V permits under the CAA.
