Under what conditions, and how, do changes occur in practices that are widely accepted and taken for granted as the right way to do things? This is the central question of this interesting and very well-written book. Kellogg addresses this question by examining efforts to change a highly institutionalized practice: the training of residents in surgery. She focuses on the implementation of a mandate from the American Council of Graduate Medical Education (2003) that limited residents to an 80-hour work week.
