This is the central question of this interesting and very well-written book. Kellogg addresses this question by examining efforts to change a highly institutionalized practice: the training of residents in surgery. She focuses on the implementation of a mandate from the American Council of Graduate Medical Education (2003) that limited residents to an 80-hour work week.
