自体软骨-软骨膜复合物行鼓室成型术后听力临床分析

摘要

Objective This paper is to explore the effect of auditory recovery of autologous cartilage-cartilage membrane complex in tympanoplasty. Methods A retrospective analysis of 65 cases of autologous cartilage-cartilage membrane complex in tympanoplasty were made,and the effect of auditory recovery as well as the situation of tympanic membrane healing,etc were observed. Results 97%long-term effective rate of tympanic membrane healing was achieved;postoperative auditory was improved,bone gap(250,500,1000,2000,4000 Hz)less than 10db;early postoperative bone gap being 250 and 500 Hz after 3 month and 6-month respectively and unobvious narrowing of low frequency region,but basically reaching 10db after 1 year;1-2 years of follow-up visit,with stable auditory test results. Conclusion The application of autologous cartilage-cartilage membrane complex in tympanoplasty can get a good auditory effect;cartilage thickness has no significant effect on hearing;autologous cartilage-cartilage membrane complex is an ideal material for tympanic membrane perforation repair.%目的：探讨自体软骨-软骨膜复合物行鼓室成型术后听力恢复效果。方法回顾性分析65例自体软骨-软骨膜复合物行鼓室成型术的患者，术后对听力恢复效果、鼓膜愈合情况等进行定期观察。结果自体软骨-软骨膜复合物进行鼓室成型术鼓膜愈合的长期有效率为97%；术后听力提高，骨气导差（250、500、1000、2000、4000Hz）均在10db以内；术后早期3个月、6个月骨气导差在250、500Hz低频区缩小不明显，但1年后基本达到10db以内；随访1~2年，听力检测结果稳定。结论采用自体软骨-软骨膜复合物行鼓室成型术可获得良好的听力效果；软骨的厚度对听力无明显影响；自体软骨-软骨膜复合物是行鼓膜穿孔修补的一种理想材料。
