缺血性脑卒中患者脑微出血的危险因素及其对认知功能的影响

摘要

目的探讨缺血性脑卒中患者脑微出血的相关危险因素及其对认知功能的影响，为临床的防治提供参考。方法采用前瞻性的研究方法，回顾性分析83例缺血性脑血管病患者的临床资料，采用梯度回波T2*WI观察，其中36例患者合并脑微出血（合并脑微出血组），47例患者未发现脑微出血（对照组），比较两组的相关危险因素及对认知功能的影响。结果单因素分析显示缺血性脑卒中患者脑微出血与合并高血压、糖尿病、高血脂以及腔隙性脑梗死数目和脑白质疏松程度有相关性（P<0.05）；多因素非条件的Logistic回归分析显示合并高血压、糖尿病、高血脂是缺血性脑血管病患者脑微出血的独立危险因素。比较两组患者对认知功能的影响，合并脑微出血组各项评分明显低于对照组，差异有统计学意义（P<0.05）。结论探讨缺血性脑卒中患者脑微出血的的相关危险因素及其对认知功能的影响，对于预测梗死后脑出血有重要的临床意义。%　　Objective To explore risk factors for cerebral microbleeds and the relationship between cerebral microbleeds and cognition disorders in patients with ischemic cerebrovascular disease, and it provides the reference for clinical. Methods By the prospective research methods,83 patients with ischemic cerebrovascular disease were retrospective analyzed from January 2010-June 2012,by gradient echo T2*WI observation,36 cases with cerebral microbleeds were as experimental group,47 cases was not found as control group,two groups of related risk factors and the influence of cognitive function were compared.Results Single factor analysis results showed that patients with cerebral microbleeds with high blood pressure,diabetes,high blood fat and number of lacunar cerebral infarction and cerebral white matter loose was a correlation degree,the difference was statistically significant (P<0.05);Many factors of Logistic regression showed that high blood pressure,diabetes,high blood fat were the independent risk factors of cerebral microbleeds.(P<0.05). The experimental group and control group were compared to the influence of cognitive function,and the various score was significantly lower than the control group,the difference was statistically significant(P<0.05). Conclusion To investigate the associated risks factors and their impact on cognitive function of patients with cerebral microbleeds has important clinical significance for predicting infarct brain hemorrhage.
