肠道病毒EV71和CA16感染患儿的体液免疫功能分析

摘要

Objective To explore the relationship between humoral immune function and clinical condition in children with intestinal virus EV71 and CA16 infection. Methods Retrospective analysis of July October 2014 to 2015 Hangzhou children's Hospital of 260 cases of hand,foot and mouth disease in children with data,80 cases of positive CA16,180 cases were positive for EV71 EV71 infection according to the illness points severe group(80 cases)and mild type(100 cases),in the same period in 100 cases of children health examination in our hospital as a healthy control group. The levels of immunoglobulin C4,IgM,IgA and complement C3 and lgG were measured by the immune assay,and the data were analyzed by U Mann-Whitney test. Results The humoral immune function in children with hand foot and mouth disease EV71 infection affected,led to the immunoglobulin lgG and IgA decreased,IgM increased and complement C3,C4 level,and related with the severity of disease. Conclusions EV71 infection in children with hand,foot and mouth disease in children's humoral immune function was affected,leading to the immunoglobulin lgG and IgA decreased,IgM and complement C3,C4 level increased,and with the severity of the disease.%目的：探讨肠道病毒EV71和CA16感染患儿的体液免疫功能与临床病情的关系。方法对2014年10月至2015年7月260例手足口病患儿资料进行回顾性分析，其中CA16阳性80例，EV71阳性180例，EV71阳性根据病情分为重症组80例和轻症型100例，以同期健康体检的儿童100例作为健康对照组。用免疫比浊法测定免疫球蛋白lgG、IgM、IgA及补体C3、C4水平，采用Mann-Whitney U检验分析数据。结果 EV71感染手足口病重症组患儿的IgG、lgM、IgA、C3、C4水平的中位数分别为7.510、1.295、0.520、1.170、0.410 g/L，轻症组患儿的IgG、lgM、IgA、C3、C4水平的中位数分别为7.985、1.460、0.580、1.120、0.420，分别与对照组间的差异具有统计学意义（P均<0.05）。其中重症组IgG、lgA低于轻症组，lgM、C3、C4高于轻症组。EV71感染手足口病组血清免疫球蛋白lgM高于CA16感染组，差异有统计学意义（P<0.05）。结论 EV71感染手足口病患儿的体液免疫功能受到了影响，导致免疫球蛋白lgG和IgA降低，IgM及补体C3、C4水平增高，且与病情的轻重相关。
