无创正压辅助通气在NRDS中的应用效果研究

         

摘要

目的:探讨无创正压辅助通气在新生儿特发性呼吸窘迫综合征中的应用效果。方法2015年1月至2016年1月特发性呼吸窘迫综合征的新生儿52例,随机分成观察组和常规组,每组各26例;两组均予积极对症治疗,在此基础上,常规组采取常规机械通气治疗,观察组采取无创正压辅助通气治疗,对比分析两组应用效果。结果治疗后,观察组的临床总有效率达100%,常规组达96.2%,差异无统计意义(P>0.05),观察组的通气时间(71.3±21.4)h、住院时间(13.4±5.7)d均低于常规组,差异有统计学意义(P<0.05),观察组在通气治疗中出现肺部感染1例,发生率3.8%,常规组有2例出现肺部感染,1例气胸,发生率11.5%,观察组明显低于常规组,差异有统计学意义(χ2=2.225,P<0.05)。结论特发性呼吸窘迫综合征的新生儿采取无创正压辅助通气治疗,可有效缩短通气时间和住院时间,且并发症发生率低,安全性高,值得推广和应用。%Objective To explore the effect of noninvasive positive pressure assisted ventilation in the treatment of neonatal idiopathic respiratory distress syndrome. Method In January 2015 to 2016 January in our hospital 52 patients with idiopathic respiratory distress syndrome in newborns as the object of study were randomly divided into study group and routine group,26 cases in each group;two groups were given symptomatic treatment actively,on the basis of this,the conventional group adopt conventional mechanical ventilation therapy,study group to take of noninvasive positive pressure ventilation therapy,comparative analysis two groups of application effect. Results Through treatment,the study group,the clinical total efficiency reaches 100%,the conventional group reached to 96.2%. There was no significant difference,with statistical significance (P>0.05),the group of ventilation time(71.3±21.4)h,hospitalization time(13.4±5.7)d were lower than normal group,the difference was statistically significant(P<0.05),the study group in the ventilation in the treatment of pulmonary infection in 1 cases,the incidence rate was 3.8%, the conventional group had 2 cases of pulmonary infection,1 case of pneumothorax,incidence rate was 11.5%,the group was significantly lower than that of the normal group,the difference has statistical significance(χ2=2.225,P<0.05). Conclusion For newborn patients with idiopathic respiratory distress syndrome of the newborn of noninvasive positive pressure ventilation therapy can effectively shorten the duration of ventilation and hospitalization time,and the complication rate is low,high safety,worthy of promotion and application.
