经皮穴位电刺激对全膝关节置换术后疼痛及活动度改善的研究

摘要

目的 探讨膝关节骨性关节炎(KOA)全膝关节置换术(TKA)后经皮穴位电刺激对其疼痛、关节活动度的改善效果.方法 选取经TKA治疗KOA患者120例,随机分为2组,各60例,对照组行常规康复治疗,观察组60例行康复治疗联合经皮穴位电刺激治疗,连续治疗6个月,比较两组治疗前后整体功能、患膝关节活动范围(ROM)、疼痛程度变化及焦虑、血栓发生率.结果 观察组治疗后HSS评分、ROM屈、伸测评结果均显著高于对照组(P＜0.05);VAS评分低于对照组(P＜0.05);观察组血栓发生率3.33％,焦虑发生率23.33％,对照组血栓发生率18.33％,焦虑发生率41.67％;两组比较差异有统计学意义(P＜0.05).结论 应用TKA治疗KOA术后行经皮穴位电刺激,方法安全,操作简单,可有效改善患者疼痛程度,改善关节活动度,同时提高整体功能,且还可实现对焦虑、血栓的有效控制,故值得推广.%Objective To investigate the effect of the electrical stimulation of the skin on the pain and the degree of joint motion in the treatment of knee osteoarthritis (KOA) after total knee arthroplasty (TKA).Methods 120 cases of patients with KOA treated with TKA in our hospital, were divided into two groups by the random number arrangement, the control group of 60 routine rehabilitation therapy, the treatment group of 60 eases of rehabilitation treatment combined with transcutaneous electrical acupoint stimulation, continuous treatment for 6 months, the overall function of the knee joint the scope of activities (ROM), the degree of pain and anxiety, changes in incidence of thrombosis were observed before and after treatment in the two groups.Results After treatment, HSS, ROM the whole function of flexion and extension evaluation results were significantly higher than control group (P＜0.05);the VAS scores were significantly lower than the control group (P＜0.05);the control group thrombosis rate was 18.33％, the incidence of anxiety was 41.67％;the treatment group thrombosis rate was 3.33％, the incidence of anxiety was 23.33％ (P＜0.05).Conclusion The use of TKA for the treatment of KOA postoperative transcutaneous electrical stimulation, safety, easy to operate, can effectively improve the patient's pain, improve joint mobility, while improving the overall function, and can achieve effective control of anxiety, thrombosis, so it is worthy of promotion.
