超声联合钼靶在乳腺肿块中的应用

             

摘要

目的 探讨超声联合钼靶在乳腺肿块中的应用效果.方法 回顾性分析2014年1月至2016年6月入院的235例病理证实确诊的乳腺肿块患者,病理活检前对所有患者行全乳腺超声扫描、双侧钼靶X线检查及超声和钼靶X线联合检查.比较三组检查方法诊断情况以及不同诊断方法在不同类型乳腺肿块漏诊情况.结果 超声组较钼靶组患者诊断符合率情况差异明显,具有统计学意义(P<0.05);联合组较钼靶组患者诊断符合率显著上升,差异具有统计学意义(P<0.05);联合组较超声组患者诊断符合率显著上升,差异具有统计学意义(P<0.05);超声组较钼靶组患者漏诊率情况差异不具有统计学意义(P>0.05);联合组较钼靶组患者漏诊率显著下降,差异具有统计学意义(P<0.05).联合组较超声组患者漏诊率情况显著,差异具有统计学意义(P<0.05);联合诊断在不同类型乳腺肿块诊断无一漏诊,超声在乳腺癌(14.3%)、纤维腺瘤(14.3%)的诊断上漏诊率较高,钼靶在乳腺癌(11.1%)、乳腺囊肿(11.1%)的诊断上漏诊率较高,2例漏诊为腺体紊乱型,1例未能准确诊断为囊肿型,3例均通过病理学确诊.结论 在乳腺肿块诊断中超声检查和钼靶X线检查是重要的诊断依据,疾病检出率均较高,符合诊断率较高,但是两者联合检查更能明显的提高符合诊断率,漏诊率极低,具较高的临床应用价值,具有借鉴性.%Objective To explore the value of application of ultrasonic combined molybdenum target in breast tumors. Methods Retrospectively analysis between January 2014 and June 2014,235 cases of hospitalized patients with pathology confirmed the diagnosis of breast lumps,pathological biopsy in all patients before the whole breast ultrasound scanning,bilateral molybdenum target X-ray and ultrasound and molybdenum target X-ray inspection. Three methods diagnosis and different diagnosis methods were compared in different types of misdiagnosis and breast lumps. Results Diagnosis coincidence rate in patients with ultrasound had statistically significant differences with mammography group (P<0.05). Joint with mammography group had a significant rise,the diagnosis coincidence rate differences had statistically significant(P<0.05). Joint group compared with ultrasonic diagnosis coincidence rate in patients had significant rise,statistically significant difference(P<0.05). Missed diagnosis in patients in ultrasound group than mammography was not statistically significant difference(P>0.05). Joint group's mammography had a significant reduction in the missed diagnosis,statistically significant difference(P<0.05).Combined group was more significantly,the missed diagnosis in patients with ultrasound group was significantly difference (P<0.05). Joint diagnosis in different types of breast lumps none in diagnosis, ultrasonography in breast cancer(14.3%),fiber adenoma(14.3%)on the diagnosis of missed diagnosis was high,molybdenum target in breast cancer(11.1%),breast cyst(11.1%)on the diagnosis of missed diagnosis was high,two cases of misdiagnosis for glandular disorder type,1 case failed to accurately diagnosed with type cyst,3 cases were confirmed by pathology. Conclusion Ultrasound and mammography in diagnosing breast lumps X-ray examination is an important diagnosis,disease detection rate is higher,in line with the diagnostic rate is higher,but the joint inspection more significantly improves in line with the diagnostic rate,missed diagnosis is extremely low,has high clinical value and worth reference.
