泻土化瘀方配合针刺治疗痰瘀内阻型顽固性失眠疗效观察

摘要

Objective:To observe clinical effects of earth-reducing stasis-removing prescription combined with acupuncture in the treatment for refractory insomnia of phlegm-stasis internal obstruction pattern.Methods:All 120 patients were randomly divided into the control group and the observation group,60 cases each group.The observation group used earth-reducing stasis-removing prescription combined with acupuncture,the decoction one dose each day,acupuncture,once each day,needling for five days and resting for two days.The control group took Estazolam tablets before sleeping,1～2 mg each day.One course of the treatment was four weeks.PSQI scales,SDS,SAS of both groups were performed before and after the treatment,clinical effects of both groups were evaluated.Results:Total effective rate of the observation group was 9322％,higher than 77.59％ of the control group (P＜0.05);the observation group was lower than the control group in the scales of five factors including sleeping quality sleeping time,sleeping efficiency,hypnotic and daily functions,and total scales after treating in PSQI scales(P＜0.01);the observation group was lower than the control group in SDS and SAS after treating (P＜0.01).Conclusion:Earth-reducing stasis-removing prescription combined with acupuncture could improve sleeping quality of the patients with refractory insomnia of phlegm-stasis internal obstruction pattern,its clinical effects are superior to these of Estazolam tablets.%目的:观察泻土化瘀方配合针刺治疗痰瘀内阻型顽固性失眠的临床疗效.方法:将痰瘀内阻型顽固性失眠患者120例随机分为对照组和观察组各60例.观察组采用泻土化瘀汤配合针刺治疗,泻土化瘀方1剂/d,针刺治疗1次/d,每针刺5天休息2天.对照组睡前服用艾司唑仑片,1～2 mg/d,疗程均为4周.治疗前后进行匹兹堡睡眠质量指数量表(PSQI)评分、抑郁自评量表(SDS)和焦虑自评量表(SAS)进行评分;同时评价2组临床疗效.结果:观察组总有效率为93.22％,高于对照组的77.59％(P＜0.05);治疗后观察组PSQI量表睡眠质量、睡眠时间、睡眠效率、催眠药物、日间功能5个因子评分和总分均低于对照组(P＜0.01);治疗后观察组SDS及SAS评分均低于对照组(P＜0.01).结论:泻土化瘀方配合针刺能改善痰瘀内阻型顽固性失眠患者的睡眠质量,临床疗效优于艾司唑仑片.
