仙灵骨葆胶囊联合甲氨蝶呤治疗类风湿性关节炎继发骨质疏松症37例

摘要

目的:观察仙灵骨葆胶囊联合甲氨蝶呤治疗类风湿性关节炎继发骨质疏松症的临床疗效.方法:将74例患者按就诊的先后顺序随机分为观察组、对照组各37例,2组入院后,均行血、尿常规以及肝肾功能检查,对照组在常规治疗的基础上口服甲氨蝶呤治疗,5~10 mg/次,1~2次/周.观察组在对照组治疗的基础上服用仙灵骨葆胶囊,3粒/次,2次/d.2组均连续治疗2个月.观察2组临床疗效及治疗前后骨密度、晨僵时间、关节压痛指数、关节肿胀指数及关节功能指数等指标的变化情况.结果:晨僵时间、关节压痛指数、关节肿胀指数及关节功能指数等指标治疗前后2组组内比较,差异有统计学意义(P<0.05);治疗后组间比较差异也有统计学意义(P<0.05).腰椎、股骨颈骨密度治疗前后2组组内比较,差异有统计学意义(P<0.05);治疗后组间比较差异也有统计学意义(P<0.05).总有效率观察组为94.6%,对照组为75.7%,2组比较差异有统计学意义(P<0.05).结论:采用仙灵骨葆胶囊联合甲氨蝶呤治疗类风湿性关节炎继发骨质疏松症临床疗效显著,且能够有效改善临床症状与体征,调节骨代谢.%Objective:To observe clinical effects of XianLing GuBao capsules combined with methotrexate in treating rheumatoid arthritis patients with secondary osteoporosis.Methods:All 74 patients were randomized into the observation group and the control group according to the visiting order,37 cases each group,after they were hospi-talized,both groups accepted blood and urine routine examination,liver and kidney function examination,the control group took methotrexate on the foundation of conventional therapy,5 to 10 mg each time,once to twice each week. The observation group took XianLing GuBao capsules, three pills each time, twice each day, both groups were treated for two months consecutively.Clinical effects,the changes of bone densinty,morning stiffness time,tender joint count, joint swelling index, joint functional index and others of both groups were observed before and after treating. Results:The difference had statistical meaning in the comparisons of morning stiffness time, tender joint count,joint swelling index,joint functional index and others within two groups before and after treating (P<0.05);the difference had statistical meaning in the comparisons between both groups after treating (P<0.05). The difference was statistically significant in the comparisons of lumbar and femoral neck bone density within two groups before and after treating(P<0.05);there was a significant difference in the comparisons between both groups after treating (P<0.05).Total effective rate of the observation group was 94.6%,higher than 75.7% of the control group, and the difference showed statistical meaning (P<0.05).Conclusion: XianLing GuBao capsules combined with methotrexatecould obtain notable effects in treating rheumatoid arthritis patients with secondary osteoporos is, which could effectively improve clinical symptoms and body signs,regulate bone metabolism.
