Objective To examine the hypothesis that functional appliances enhance mandibular growth in the treatment of skeletal Class Ⅱ malocclusion. Methods A literature search was performed in electronic databases according key words "skeletal Class Ⅱ malocclusion" , and full texts of selected articles were collected by hand-made method.The data were extracted from the articles. The quality of the trials was evaluated and meta-analysis method was used to analyze the data through RevMan 4.2. Results 19 articles could be qualified for the final analysis. Those indices including SNB (Activator, Twin-block, Herbst group), Go-Gn, Co-Pg, Co-Gn, Co-Go, demonstrated significant effect of functional appliances on mandibular growth in comparing with those of untreated control group. There were no significant differences between the two groups for the indicias of SNB (Fr(a)nkel-Ⅱ) and Go-Me. Conclusion Functional appliances can enhance mandibular growth in treatment of skeletal Class Ⅱ malocclusion. This mainly dues to the growth of the ramus instead of changes of mandibular body length.%目的 系统分析经过功能性矫治器治疗的骨性Ⅱ类患者下颌骨的生长变化是否大于未治疗的Ⅱ类错患者.方法 以骨性Ⅱ类错(牙合)为关键词,先通过网络数据库检索后再用手工方法查找原文献,获得全文.从收集到的符合纳入和排除标准的文献中提取需要的数据资料,对其进行质量评估,并采用RevMan 4.2软件进行Meta分析.结果 符合纳入和排除标准的文献共19篇.Meta分析显示:SNB(Activator组,Twin-block组,Herbst组)、Go-Gn、Co-Pg、Co-Gn、Co-Go与未治疗组相比,其增加有统计学意义,而SNB(Frankel-Ⅱ组)、Go-Me差异无统计学意义.结论 功能矫治器可以增加下颌的生长量,刺激下颌向前生长,而下颌的生长主要是由下颌升支的生长造成的,下颌体绝对长度无明显改变.
