目的：分析癌症住院患者住院费用、不同支付方式的费用差别及影响因素，为完善医保支付政策提供参考依据。方法收集沈阳军区总医院自2004年1月至2013年12月住院癌症患者的住院天数、住院费用、自负费用等相关信息，进行统计分析。结果医保与非医保癌症患者就医人数整体均呈上升趋势，医保患者比例不断增加，但总体上非医保患者就医人数仍多于医保患者；癌症患者整体住院天数有所缩短，而医保与非医保患者住院时长的差距也逐渐缩小；医保与非医保患者的均次住院费用均呈上升趋势，二者之间差异有统计学意义(P <0．05)；医保患者平均单次住院的自负费用总体呈上升趋势，且与政策调控密切相关。结论随着癌症治疗日趋规范化，支付方式对患者治疗方案的影响正逐渐弱化，而医保政策对控制住院费用增长，减轻癌症患者经济负担，合理分配医疗资源等方面发挥了一定作用，正逐步成为患者支付医疗费用的重要方式。%Objective To explore the analysis of cancer inpatient hospital costs under different methods of payment and in-fluencing factor to formulate perfecting medical insurance payment policy as reference. Methods Collect and statistic anal-ysis relevant information such as length of hospital stay,hospitalization cost and self-pay cost for our hospital in Shenyang since 2004 to 2013. Results The number of cancer inpatients of different payment types both present rising trend,the med-ical insurance ratio of inpatients was increased rapidly. However, medicare population were still less than those with ex-pense,length of hospital stay became shorter while the gap got narrow,an uptrend could be observed of hospitalized expense and the difference was remarkable,the hospitalization cost of medical insurance inpatient was increasing and closely related with control policy. Conclusion Along with normalization of cancer therapy,the influence of different methods of payment on treatment protocols is gradually getting weak,medical insurance payment policy has impact on controlling hospitalization cost,lightening financial burden of cancer patient,as well as allocating medical resources in a reasonable way. Therefore,it is becoming an important defray pattern of hospitalization cost.
