Objective To explore the effect of hepatic transcatheter arterial chemoembolization( TACE) combined with ster-eotactic radiation therapy in primary liver cancer. Methods A retrospective study was performed on 56 patients with large liver cancer admitted in No. 463 Hospital of PLA from March 2010 to March 2014. Patients were divided into the combina-tion therapy group(n=29)and the radiation therapy group(n=27). Patients in the combination therapy group were given TACE combined with stereotactic radiation therapy,and the others in the radiation therapy group were given single stereotac-tic radiation therapy. The short-term effect and long-term survival rate before and after the treatment were compared between the two groups. Results The short-term total effective rate of combination therapy group was 58. 6%,which was obviously higher than that of radiation therapy group ( 44. 4%) , the differences between the two groups were statistically significant (P<0. 05). The survival rate in the combination therapy group and radiation therapy group at 6,12,18 months were 82. 8% versus 66. 7%,58. 6% versus 44. 4%,44. 8% versus 29. 6%,the differences between the two groups were statisti-cally significant(P<0. 05). There were slightly adverse reactions,and all the cases recovered after conventional treatments. Conclusion TACE combined with stereotactic radiation therapy can improve the short-term effect and long-term survival rate of patients with primary liver cancer,and has higher safety.%目的：探讨经导管肝动脉化疗栓塞术( TACE)联合立体定向放疗与单纯立体定向放疗治疗较大原发性肝癌的临床疗效。方法选取2010年3月至2014年3月解放军463医院收治的56例经确诊为大肝癌的患者为研究对象，按照患者及家属意愿将患者分为联合治疗组(n=29)和放疗组(n=27)。联合治疗组患者给予TACE联合立体定向放疗，放疗组患者给予单纯立体定向放疗。比较两组患者治疗前、后的近期疗效及远期存活率。结果联合治疗组近期治疗总有效率为58．6%，明显高于放疗组的44．4%，两组间比较，差异有统计学意义( P<0．05)。随访6、12、18个月，联合治疗组患者的存活率分别为82．8%、58．6%、44．8%；放疗组的分别为66．7%、44．4%、29．6%；两组间比较，差异均有统计学意义(P<0．05)。两组患者的不良反应多轻微，经保守治疗后均恢复正常。结论 TACE联合立体定向放疗能提高较大原发性肝癌患者的近期疗效及远期存活率，且安全性好。
