Objective: To evaluate the clinical value of left atrial f'unction by strian rate imaging (SRI) in primary hypertensive patients. Methods: Eighty-nine patients with primary hypertensions were divided into two groups, normal geometry group (LVN, n=40) and remodeling geometry group (LVR, n=49). Forty-one healthy participants were included as the control. At four-chamber view, the strain rate curves were gained by SRI in the middle segments of each left atrial lateral wall .Strain rate parameters SRs, SRe and SRa were measured during left ventrial systolic phase, during left ventrial diastolic early phase and later phase, respectively. The peak velocity during left ventrial diastolic early phase and later phase (Ea and Aa) at the left ventricle lateral wall of annulus mitral valve was measured by tissue Doppler imaging (TDI), and then Ea/Aa was calculated. The values of E peak and A peak velocity (E and A) of bicuspid were measured by pulsed wave Doppler (PW), and then E/A was calculated. The left anteroposterior diameter (LAD) was measured by 2D Doppler. Results: Compared with the control group, the values of SRs and SRe were reduced in the two hypertensive groups, meanwhile the value of SRa was higher in LVN group (P ＜ 0.01). The value of Ea/Aa was reduced in the two hypertensive groups, but E/A was reduced in LVR group (P ＜ 0.01). The value of LAD was higher in hypertensive groups. And the value of LAD was higher in LVR group than that in LVN group (P ＜ 0.01). Conclusion: The values of Sra, SRe and SRs can be used to assess the left atrial systolic and diastolic functions.%目的:探讨应变率成像(SRI)评价高血压患者左房功能的临床价值.方法:原发性高血压病患者89例,分为左室正常构型组(LVN)40例和左室重构组(LVR)49例;正常人41例为对照组.于心尖四腔切面,应用SRI获取左房侧壁中段应变率曲线,分别测量左室收缩期、舒张早期、舒张晚期左房峰值应变率(SRs、SRe、SRa);应用组织多普勒成像(TDI)获得二尖瓣环附着处左室舒张早期及舒张晚期峰值速率(Ea、Aa),计算Ea/Aa.应用脉冲多普勒(Pw)测量二尖瓣E峰及A峰流速(E、A),计算E/A;应用二维超声测量左房前后径(LAD).结果:与对照组比较,高血压各组SRe及SRs均减低,LVN组SRa增高(P<0.01);高血压各组Ea/Aa均减低,LVR组E/A减低(P<0.01);高血压各组LAD均增高,且LVR组大于LVN组(P<0.01).结论:SRa、SRe、SRs能够评价高血压病患者左房收缩及舒张功能.
