超声体检对早期发现无症状肝癌和小肝癌的临床诊断价值研究

         

摘要

目的 探讨超声体检对早期发现无症状肝癌和小肝癌的临床诊断价值.方法 选取该院2016年9月—2018年7月收治的80例肝癌患者为研究对象,均经病理检查确诊,根据就诊时有无症状分为有症状组(n=27)和无症状组(n=53),选择彩色多普勒超声进行检查,对比有无症状组的诊断准确率和有无症状组的诊断准确率.结果 CT平扫检查肝癌检出率为76.25%(61/80);超声检查肝癌检出率为93.75%(75/80);CT增强扫描检查肝癌检出率为96.25%(77/80);CT增强扫描检查肝癌检出率稍高于超声检查,但差异无统计学意义(χ2=0.386 6,P>0.05);超声和CT增强扫描检查肝癌检出率明显高于CT平扫,差异有统计学意义(χ2=12.009 8、16.864 3,P<0.05).有症状组:超声检查肝癌检出率明显高于CT平扫,差异有统计学意义(χ2=20.410 0,P<0.05);无症状组:超声和CT增强扫描检查肝癌检出率明显高于CT平扫,差异有统计学意义(χ2=20.014 3、9.810 4,P<0.05).大肝癌组:三种检查方法肝癌检出率对比差异无统计学意义(χ2=3.381 3、0.000 0、3.381 3,P>0.05);小肝癌组:超声和CT增强扫描肝癌检出率明显CT平扫,差异有统计学意义(χ2=18.482 4、29.420 4,P<0.05).结论 在体检中应用超声检查能帮助发现小肝癌,超声检查是肝癌诊断的有效方法.%Objective To study the clinical diagnosis value of ultrasound physical examination in early discovery of asymptomatic liver cancer and small liver cancer. Methods 80 cases of liver cancer patients admitted and treated in our hospital from September 2016 to July 2018 were selected and all patients were confirmed by the pathological test, and the patients were divided into the symptom group with 27 cases and the non-symptom group with 53 cases, and the color Doppler ultrasound was used for examination, and the accuracy was compared. Results The liver cancer test rate by CT plain scanning, the ultrasound examination and by the CT enhanced scanning were respectively 76.25% (61/80), 93.75% (75/80), 96.25% (77/80), and the test rate of liver cancer examined by the CT enhanced scanning was slightly higher than that of ultrasound examination, but the difference was not obvious, without statistical significance (χ2=0.3866, P>0.05); and the test rate of liver cancer by the ultrasound and CT enhanced scanning was obviously higher than that of CT plain scanning, and the difference was statistically significant (χ2=12.009 8, 16.864 3, P<0.05), and the test rate of liver cancer by the ultrasound examination was higher than that of CT plain scanning, in terms of symptom group, and the difference was statistically significant (χ2=20.410 0, P<0.05); and the test rate of liver cancer by the ultrasound and CT enhanced scanning was obviously higher than that of CT plain scanning, and the difference was statistically significant (χ2=20.014 3, 9.810 4, P<0.05), and the test rate of liver cancer of the three examination methods in the big liver cancer group was not obvious, without statistical significance (χ2=3.381 3, 0.000 0, 3.381 3, P>0.05), and the test rate of liver cancer by the ultrasound and CT enhanced scanning in the small liver cancer group was obviously higher than that of CT plain scanning, and the differences were statistically significant (χ2=18.482 4, 29.420 4, P <0.05). Conclusion The application of ultrasound examination in the physical examination can help discover the small liver cancer, and the ultrasound examination is an effective diagnosis way of liver cancer.
