结核病高发地区18F-FDG PET/CT双期扫描诊断孤立性肺结节

         

摘要

he purpose was to evaluate whether the dual time point of"F-Fluorodeoxyglucose ( 18F-FDG ) positron emission tomography/computed tomography (PET/CT) can be helpful for differentiating between malignant and benign pulmonary lesions. Methods 105 patients with a solitary pulmonary nodule (SPN) were studied prospectively. PET/CT images were obtained one hour and two hours after intravenous injection of"F-FDG. A receiver-operating characteristic (ROC) analysis was performed to evaluate the delayed imaging. Results The accuracy for delayed imaging was 78.0%. There was significant o-verlap in retention index (RI) between benign and malignant group (P >0. 05). The accuracy for RI was 67. 0% and the area under curve( AUC) was 0. 503 . Conclusion This study indicates that dual-time-point" F-FDG PET/CT is not indicated to differentiate between malignant and benign pulmonary lesions in areas with a high incidence of tuberculosis.%目的 评价18F-氟代脱氧葡萄糖(18F-FDG)正电子发射性计算机断层/电子计算机断层(PET/CT)双期扫描是否有助于鉴别良恶性孤立性肺结节(SPN).方法 选择了105例SPN患者为研究对象,所有患者均分别进行早期(注射药物后1h)及延迟期(注射药物后2h)PET/CT显像,计算滞留指数(RI),通过受试者工作特性(ROC)曲线,评价延迟显像的诊断价值.结果 延迟显像诊断的准确性为78.0%,良恶性SPN的RI间存在明显的重叠,差异无统计学意义(P>0.05).RI诊断的准确性为67.0%,曲线下面积(AUC)为0.503.结论 延迟显像并不能提高PET/CT对SPN良恶性鉴别诊断的能力.
