微创钢板内固定治疗尺骨骨折的疗效分析

         

摘要

目的 探讨微创经皮钢板接骨术(MIPPO)结合镇定加压钢板(LCP)内固定治疔尺骨骨折的手术途径及其临床疗效.方法 选取我院尺骨骨折患者60例,随机分为镇定加压钢板微创经皮钢板接骨术治疗组(Ⅰ组),交锁髓内钉治疗组(Ⅱ组)及手法整复小夹板治疗组(Ⅲ组),各组20例.并通过术后回访对比各组术式的临床疗效.结果 所有患者切口均一期愈合,均未出现张力性水疱及骨筋膜室综合征的情况,其中Ⅱ组3例发生断钉、断棒和内固定松动,2例发生针道感染;Ⅲ组有2例发生延迟愈合或不愈合,并有1例导致创伤性关节炎;三组经肿关节HHS评分标准及腕关节Gartland-Werley评分系统评定各组各项指标术前与术后3、6、12个月各样本均数之间两两比较P<0.05,均具有统计学意义;术后症状改善情况显示Ⅰ组临床疗效显著优于Ⅱ组、Ⅲ组,各组结果差异具有统计学意义(P<0.05).结论 经镇定加压钢板微创经皮钢板接骨术治疗尺骨骨折疗效确切,预后理想,值得临床推广使用.%To explore the minimally invasive percutaneous plate ostleosynthesis (MIPPO ) combined with locking compression plate (LCP) internal fixation for the treatment of ulna fracture operation way and its clinical effects. Methods 60 cases of patients with fracture of the ulna in our hospital, were randomly divided into the locking compression plate fixation of minimally invasive percutaneous plate osteoeynthesis for the treatment group (group I), interlocking intramedullary nail in the treatment group (group Ⅱ) and manipulative reduction and small splint treatment group (group Ⅲ), each group 20 cases. And through the postoperative visit each postoperative curative effect contrast. Results All the patients incision healed in one stage, then are no tension blisters and the osteofascial compartment syndrome, including 3 cases of group II broken nails, broken rods and fixation loosening, 2 cases of pin track infection; group Ⅲ 2 cases of delayed union or nonunion, and in 1 patients there was the result of a traumatic arthritis; 3 groups of the elbow joint and wrist Gartland-Werley HHS score standard score system evaluation indexes were preoperative and postoperative 3 months, 6 months, 12 months each sample were with statistical significance( p < 0.05 ) ; postoperative improvement of symptoms showed a significant clinical effect of group I better than in group Ⅱ, group Ⅲ, the group difference has statistics significance (P<0.05). Conclusion The locking compression plate fixation of minimally invasive percutaneous plate osteosynthesis for the treatment of ulna fracture can lead a considerable curative effect, as well as an ideal prognosis for patients,which is highly worthy of clinical use.
