术前不同方式和时机导尿对全麻男性患者舒适度的影响

摘要

目的：探讨不同方式和时机导尿对全麻男性患者舒适度的影响。方法300例择期行全麻手术的男性患者,随机分为3组,每组100例。Ⅰ组：全麻诱导前用硅油涂尿管行导尿术；Ⅱ组：全麻诱导后用硅油涂尿管行导尿术；Ⅲ组：全麻诱导后用复方利多卡因乳膏涂尿管行导尿术。观察并比较三组患者导尿术前、中、后的血压、心率变化；对麻醉苏醒期患者进行与导尿舒适度和效果紧密相关的躁动综合评分( RS评分),并比较评分的组间差异情况。结果Ⅰ组导尿术中、术后血压、心率比术前明显升高,差异有统计学意义(P<0．05),而Ⅱ、Ⅲ组与术前比较差异无统计学意义(P>0．05)。与Ⅰ组比较,Ⅱ组RS评分显著升高,差异有统计学意义(P<0．05)；而Ⅲ组RS评分与Ⅰ组比较,差异无统计学意义(P>0．05)。结论全麻诱导后用复方利多卡因乳膏导尿(Ⅲ组)能减少患者因隐私暴露、疼痛不适导致血压、心率升高,且对降低男性患者苏醒期躁动。%Objective To investigate the impacts of different methods and timing of catheterization on comfort of male pa-tients with general anesthesia. Methods 300 patients undergoing elective general anesthesia, were randomly divided into 3 groups,100 cases in each group. Group I:conduct catheterization with silicone oil coated catheter before induction of general anes-thesia;group II:conduct catheterization with silicone oil coated catheter after induction of general anesthesia;group III: conduct catheteriaztion with compound lidocaine cream coated catheter after induction of general anesthesia. Blood pressure and heart rate changes of the three groups were observed and compared before as well as after catheterization. Patients in anesthesia recovery period were conducted restlessness score ( RS) which is related to catheterization comfort and effect,and compare the score differences be-tween groups. Results As for catheterization of group I,intraoperative as well as postoperative blood pressure and heart rate were significantly higher than preoperation situation,with a statistically significant difference(P<0. 05),but there was no statistically sig-nificant difference between those of Group Ⅱ together with Group Ⅲ and their preoperative situation ( P>0. 05 ) . Compared with group I,RS score of group II increased significantly and the difference was statistically significant(P<0. 05);but when comparing RS score of group III and group I,the difference was not statistically significant(P>0. 05). Conclusion Conducting catheteriza-tion with compound lidocaine cream after induction of general anesthesia( Group III) can reduce the incidence of raised blood pres-sure and heart rate of patients generated by privacy exposure,pain and discomfort as well as the restlessness of male patients during recovery period.
