31例高频振荡通气抢救新生儿呼吸衰竭临床研究

             

摘要

目的 探讨高频振荡通气(HFOV)治疗新生儿呼吸衰竭的疗效及安全性.方法 观察31例应用HFOV患儿治疗前后氧合通气指标、呼吸机参数的变化及临床疗效.结果 与治疗前相比,HFOV治疗后6h患者PaCO2迅速下降,差异有统计学意义(P<0.05)[(70.75±16.78)mmHg vs.(48.25±8.55)mmHg];12h的PaO2明显上升,差异有统计学意义(P<0.05)[(41.65±11.25)mmHg vs.(60.15±16.32)mmHg;吸入氧浓度(FiO2)、血氧合指数(OI)随时间降低.治愈24例,出现颅内出血等并发症5例.结论 HFOV是治疗新生儿呼吸衰竭的安全有效的方法.%Objective To investigate the efficacy and safety of high frequency oscillatory ventilation( HFOV) in the treat-ment of neonatal respiratory failure. Methods Before and after HFOV treatment,the changes of the parameters of oxygen,the pa-rameters of the ventilator and the clinical curative effect of 31 cases of sick newborn were observed. Results Compared with before treatment,PaCO2 after 6h treatment in patients with HFOV decreased rapidly,and the difference was statistically significant( P<0. 05)[(70. 75 ± 16. 78)mmHg vs. (48. 25 ± 8. 55)mmHg ]. PaO2 in 12h significantly increased,and the difference was statisti-callysignificant(P<0.05)[(41.65±11.25)mmHgvs.(60.15±16.32)mmHg.Theoxygenconcentration(FiO2)oftheblood oxygenation index( OI) decreased with time. There were 24 cases cured,5 cases of complications such as intracranial hemorrhage. Conclusion HFOV is a safe and effective method in the treatment of neonatal respiratory failure.
