AO 解剖型踝支持钢板对股骨远端 C3型骨折患者膝关节功能的影响

摘要

目的：探讨AO解剖型踝支持钢板对股骨远端C3型骨折患者膝关节功能的影响。方法选取我院2012年5月至2014年5月收治的84例股骨远端C3型骨折患者为研究对象，将所有患者按随机数字表法分为试验组（42例）和对照组（42例），对照组采用逆行交锁髓内钉进行治疗，试验组采用A O解剖型踝支持钢板进行治疗，比较2组患者治疗后的膝关节功能评分，手术时间、术中出血量、住院时间、骨折临床愈合时间，以及术后感染、膝关节疼痛、畸形愈合、延迟愈合等并发症的发生率。结果治疗后2组患者的膝关节评分等级比较差异具有统计学意义（ P＜0.05），试验组和对照组治疗后膝关节功能评分优良率分别为88％和62％，差异具有统计学意义（P＜0.05）；试验组患者手术时间、术中出血量、骨折临床愈合时间显著低于对照组（P＜0.05），试验组患者住院时间与对照组比较，差异无统计学意义（P＞0.05）；术后并发症总发生率试验组显著低于对照组（P＜0.05）。结论 AO解剖型踝支持钢板可以有效地改善股骨远端C3型骨折患者膝关节功能，缩短手术时间和骨折临床愈合时间，减少术中出血量，降低术后并发症的发生率，在临床应用上有推广价值。%Objective To explore the effects of AO anatomical ankle support plate on knee joint function in patients with type C3 fractures of distal femur .Methods Eighty‐four cases of elderly patients with type C3 frac‐tures of distal femur in our hospital from May 2012 to May 2014 were selected as the research subjects ,who were devided into experimental group(42 cases) and control group(42 cases) .The control group were treated with retro‐grade interlocking intramedullary nail ,and the experimental group were treated with AO anatomical ankle support plate .The scores of knee function after operation ,the operation time ,amount of bleeding during operation ,length of hospital stay ,clinical healing time of fractures ,and the incidence of complications such as postoperative infec‐tion ,knee pain ,deformity union ,delayed union of patients in the two groups were compared .Results After treat‐ment ,the difference of level comparision in the scores of knee function between the two groups was statistically significant (P<0.05) ,of which the excellent and good rate in the experimental group and the control group were 88% and 62% respectively ,and the difference was statistically significant (P<0.05) .The operation time ,amount of bleeding during operation and clinical healing time of fractures in the experimental group were significantly low‐er than those in the control group (P<0.05) ,of which the length of hospital stay was not significantly different from that of the control group (P>0.05) .The incidence of postoperative complications in the experimental group was significantly lower than that in the control group (P<0.05) .Conclusion The AO anatomical ankle support plate can effectively improve the knee joint function of patients with type C 3 fractures of distal femur ,shorten op‐erative time and clinical fracture healing time ,reduce the amount of bleeding during the operation and the incidence of postoperative complications ,which has popularization and application value in clinical practice .
