聚髌器与克氏针张力带治疗老年髌骨粉碎性骨折的疗效比较

摘要

Objective To compare the patellar concentrator with the kirschner wire tension band for the treatment of senile comminuted patellar fracture. Methods A retrospective analysis of our hospital from January 2008 to January 2012 was used in the patellar concentrator and kirschner wire tension band for the treatment of 87 cases of senile comminuted patellar fracture patients. To compare the clinical efficacy of the two treatment methods from the surgery, postoperative complications and efficacy. Results All the patients were followed up for an average of 11. 7 months (9-15 months) , the follow-up rate was 100%. The operative time and intraoperative blood loss in the two groups were not statistically different (P >0.05) . The time of fracture healing and the treatment cost in the two groups was statistically different (P <0. 05) . The total incidence of complications in the patellar concentrator was lower than that in the kirschner wire group (P <0. 05). The postoperative hip score in the patellar concentrator group was better than that in the kirschner wire group ( P < 0.05 ). Conclusions The patellar concentrator for the treatment of senile comminuted patella fracture has the advantages of reliable fixation, short fracture healing time, less postoperative complications, excellent functional recovery, and so on. It is an ideal clinical treatment for senile comminuted patellar fracture.%目的 比较聚髌器与克氏针张力带治疗老年髌骨粉碎性骨折的临床疗效.方法 回顾性分析我院自2008年1月～2012年1月采用聚髌器与克氏针张力带治疗的87例老年髌骨粉碎性骨折患者.从术中情况、术后并发症及疗效等方面对两种治疗方法的临床疗效进行比较.结果 所有患者经过9 ～15个月随访,平均11.7个月,随访率100％.两组患者在手术时间和术中出血量方面无统计学差异(P＞0.05)；骨折愈合时间、治疗费用方面两组比较有统计学差异(P＜0.05)；聚髌器组并发症总发生率低于克氏针张力带组(P＜0.05)；聚髌器组术后髋关节综合评分明显优于克氏针张力带组(P＜0.05).结论 聚髌器对治疗老年髌骨粉碎性骨折具有固定牢固、骨折愈合时间短、术后并发症少及功能恢复好等特点,是治疗老年髌骨粉碎性骨折的理想内固定.
