重症哮喘患者血清瘦素水平与哮喘严重程度的关系

摘要

目的 观察重症哮喘患者血清瘦素水平与哮喘严重程度的相关性,探讨瘦素在哮喘发病中的作用.方法 选择60例哮喘患者,根据哮喘严重程度分为轻、中、重度3个组,选取健康成人志愿者20例作为对照组.采用ELISA法检测各组清晨空腹血清瘦素水平,同时检测超敏C反应蛋白(hsCRP)、总血清IgE水平,并行肺功能检查.结果 哮喘患者血清瘦素水平明显高于对照组(P＜0.01).轻、中、重度组血清瘦素水平依次升高,重度组血清瘦素水平明显高于轻、中度组(P均＜0.05).中度组血清瘦素水平高于轻度组,但差异无统计学意义(P＞0.05).相关性分析显示,血清瘦素水平与女性BMI呈正相关(r=0.54,P＜0.01),与哮喘患者血清hsCRP呈正相关(r=0.336,P＜0.01),而与支气管舒张剂前第1秒用力呼气量占预计值的百分比呈负相关(r=-0.31,P＜0.05).结论 血清瘦素水平与成人哮喘临床严重程度相关,瘦素可能在哮喘的病理过程中发挥重要作用,检测血清瘦素水平对判断哮喘临床严重程度有一定的预测价值.%Objective To observe the correlation of serum Leptin levels in patients with severe asthma, and explore the role of leptin in the pathogenesis of asthma. Methods A total of 60 asthma patients and 20 health volunteers were included. The asthma patients were divided into mild, moderate and severe groups according to the severity of asthma. The Leptin levels were determined in the serum using ELISA and simultaneously high sensitivity C-reactive protein (hsCRP) , total serum IgE levels were detected, as well as did the concurrent pulmonary function tests in all asthma patients. Results The Leptin levels in asthma patients were higher compared with health control (P<0.01). In mild, moderate and severe asthma groups, serum Leptin levels were increased successively and serum Leptin levels in severe asthma group were significantly higher than those in mild to moderate asthma group (P <0. 05). The moderate group levels of serum leptin were higher than the mild group. The correlation analysis showed that serum Leptin levels and female body mass index ( BMI) showed a positive correlation ( r = 0. 54, P < 0. 01 ) , positively correlated with hsCRP ( r = 0. 37 , P < 0. 01 ) as well, but showed a negative correlation with FEV1 % pred (r = — 0. 31, P < 0. 05). Conclusions There is a relationship between the serum Leptin levels and clinical severity of asthma in adults. Serum Leptin levels in the severe asthma patients are higher than those in mild to moderate persistent asthma patients. The Leptin may play an important role in the pathogenesis of asthma, and there is some value to measure the serum Leptin levels in predicting clinical severity of asthma in a-dults.
