妊娠期糖尿病对新生儿脐血抵抗素水平及胰岛素抵抗的影响

摘要

目的：探讨妊娠期糖尿病（ GDM ）对新生儿脐动脉血清抵抗素与胰岛素抵抗的影响。方法对33例GDM母亲分娩的新生儿（观察组）及30例无糖尿病母亲分娩的足月新生儿（对照组）脐血血清抵抗素、血糖（ PG）、胰岛素进行测定，计算胰岛素抵抗指数（ IRI）。分析新生儿脐血清抵抗素与PG、胰岛素及IRI的相关性。结果观察组PG、胰岛素和IRI均明显高于对照组，抵抗素水平低于对照组（P均＜0．05）。观察组新生儿脐血血清抵抗素与PG、IRI呈正相关（ r分别为0．43、0．56，P均＜0．05），与胰岛素水平无关（P＞0．05）。结论糖尿病母亲分娩的新生儿存在胰岛素抵抗，抵抗素可能参与了胰岛素抵抗的病理过程。%Objective To investigate the effects of gestational diabetes mellitus ( GDM) on umbilical artery serum re-sistin and insulin resistance (IR) in infants.Methods The concentrations of resistin, plasma glucose (PG), insulin ( INS) and the insulin resistance index ( IRI) of the umbilical cord serum were separately examined in 33 infants of GDM mothers ( observation group ) and 30 normal control subjects ( control group ) .The correlations between serum resistin and PG, INS and IRI were analyzed .Results The umbilical artery serum levels of PG , INS and IRI in the observation group were significantly higher than those of the control group (all P<0.01), but resistin level was significantly decreased as compared with that of the control group (P<0.01).Correlation analysis showed that resistin was positively correlated with PG and IRI (r=0.43, r=0.56;all P<0.01), and no correlation was found between resistin and insulin (r=0.18, P>0.05).Conclusions The infants who were born by GDM mothers have insulin resistance , and resistin may play a role in the pathogenesis of insulin resistance .
