不同切口超声乳化联合小梁切除术治疗白内障合并青光眼临床疗效对比研究

摘要

目的:探讨超声乳化术联合小梁切除术不同切口治疗白内障合并青光眼的疗效.方法:选取88例青光眼合并白内障患者,随机分为对照组(44例),予以单切口超声乳化术联合小梁切除术,观察组44例,予以双切口超声乳化术联合小梁切除术,观察两组疗效.结果:观察组患者的临床有效率稍高于对照组,但组间比较无统计学差异(P>0.05);治疗后,两组患者眼内压均显著改善,而观察组改善更显著(P<0.05);同时治疗后,两组裸眼视力及最佳矫正视力均显著改善,而观察组较对照组改善更明显(P<0.05);两组在功能型滤过泡上比较差异具有统计学意义(P<0.05);此外,治疗后,相较于对照组,观察组角膜内皮细胞变异系数显著降低(P<0.05),在并发症发生情况上,两组比较无统计学差异(P>0.05).结论:双切口白内障超声乳化联合小梁切除术治疗白内障合并青光眼能有效控制眼内压,促进功能性滤过泡形成和视力恢复,并发症发生率低.%Objective:To explore the clinical effect of phacoemulsification and trabeculectomy with different approaches in the treatment of cataract complicated with glaucoma.Methods:88 cataract patients with glaucoma trea-ted from March 2016 to March 2017 in our hospital were selected.The subjects were randomly divided into two groups,44 cases each group.The control group received the phacoemulsification and trabeculectomy with single ap-proach;the observation group received the phacoemulsification and trabeculectomy with double approaches.The clinical effect was observed.Results:Compared with control group,the clinical effective rate for observation group was not statistically increased(P>0.05);after treatment,the intraocular pressure for observation group was even better than control group(P<0.05);after treatment,the uncorrected visual acuity and best corrective visual acuity for observation group were even better than control group(P<0.05).Conclusion:The phacoemulsification and trab-eculectomy with double approaches can efficiently control the intraocular pressure,promote the formation of func-tional filtering bleb and vision recovery and reduce the incidence rate of complications.
