利多卡因与不同剂量曲马多预处理缓解丙泊酚注射痛的效果比较

摘要

目的 对比研究不同剂量曲马多和利多卡因预处理缓解异丙酚注射痛的效果.方法 选择我院2010年1月至2011年2月250例ASA分级Ⅰ～ Ⅱ级、接受门诊手术的女性患者,按随机数字表法分为5组各50例.T50、T100和T150组分别静脉预注射曲马多50、100和150 mg,L50组静脉预注射利多卡因50 mg,对照组(C组)接受等渗盐水静脉预处理,1分钟后注射异丙酚,注射期间采用四分法评估患者的疼痛程度.结果 T50、T100、T150和L50组异丙酚注射痛发生率均明显低于C组,差异均有统计学意义(P＜ 0.05)；L50和T50组异丙酚注射痛发生率比较,差异无统计学意义(P＞0.05)；T100和T150组异丙酚注射痛发生率明显低于L50和T50,差异均有统计学意义(P＜0.05)；T100和T150组异丙酚注射痛发生率比较,差异无统计学意义(P＞0.05).结论 曲马多与利多卡因预处理均能明显减轻患者的异丙酚注射痛发生率.50 mg曲马多和50 mg利多卡因减轻异丙酚注射痛具有同样的效果,曲马多100 mg和150 mg预处理具有更好的效果,但两者之间无明显差异.%Objective The aim of this study was to compare the efficacy of pretreatment with different doses of Tramadol and Lidocaine for prevention of Propofol induced pain. Methods From Jan 2010 to Feb 2011 ,250 female patients in grade I or II (ASA) , scheduled to undergo outpatient surgery,were randomly assigned to five groups (50 cases in each group). Group T50,group T100 and group T150 received Tramadol 50 mg,Tramadol 100 mg and Tramadol 150 mg respectively. Group L50 received Lidocaine 50 mg. And the control group, group C, received pretreatment with isotonic saline solution, and Propofol 1 min later. Four-Point Scale was used to assess the pain of patients during the injection of Propofol. Results The incidence of Propofol injection pain in group T50,T100,T150 and L50 were significantly lower than that in group C (P < 0.05) ,that was not significantly different between group L50 and group T50 (P > 0. 05) , those in group T100 and T150 were significantly lower than those in group L50 and group T50 (P < 0.05) ,that was not significantly different between group T100 and group T150 (P > 0. 05). Conclusions The pretreatment with Tramadol and Lidocaine can significantly reduce the incidence of Propofol injection pain. Tramadol 50 mg and Lidocaine 50 mg are equally effective in attenuating pain during intravenous injection of Propofol compared with the former. Difference between the two methods is not significant.
