超声乳化联合房角分离术治疗急性原发性闭角型青光眼的临床疗效分析

摘要

目的 探讨超声乳化联合房角分离术治疗急性原发性闭角型青光眼的临床疗效.方法 298例急性原发性闭角型青光眼单眼患者按随机数字表法分为两组,房角分离组采用超声乳化联合房角分离术,小梁切除组采用超声乳化联合小梁切除术,比较两组眼科相关参数、并发症与远期生活质量.结果 房角分离组术后BCVA、CAC、房角、AOD500与TIA水平均高于小梁切除组,IOP、PAS水平、虹膜纤维素样渗出率及术后浅前房率低于小梁切除组;术后6个月躯体功能、社会功能与总体健康评分高于小梁切除组(P<0.05).结论 超声乳化联合房角分离术治疗急性原发性闭角型青光眼的临床疗效显著,安全性较高,具有借鉴意义.%Objective To investigate the clinical effect of phacoemulsification combined with goniosynechialysis in the treat-ment of acute primary angle-closure glaucoma. Methods Two hundreds and ninety-eight patients with acute primary angle-closure glaucoma were randomly divided into goniosynechialysis group and trabeculectomy group.The goniosynechialysis group was treated with phacoemulsification combined with goniosynechialysis.The trabeculectomy group was treated with phacoemulsification combined with trabeculectomy.Ophthalmology related parameters,complications and long-term quality of life were compared between the two groups. Results The postoperative BCVA,CAC,angle,AOD500 and TIA levels were significantly higher while the levels of IOP and PAS,exu-dation rate and rate of shallow anterior chamber were significantly lower in the goniosynechialysis group than those in the trabeculectomy group.After 6 months of operation,physical function,social function and general health score were higher in the goniosynechialysis group than those in the trabeculectomy group (P <0.05).Conclusion Phacoemulsification combined with goniosynechialysis in the treatment of acute primary angle-closure glaucoma have significant clinical effect and high safety.It is of great value to the clinicians.
