小儿肺炎支原体肺炎心血管系统损伤的影响因素研究

摘要

目的 探讨小儿肺炎支原体肺炎(MPP)心血管系统损伤的影响因素.方法 选取2014-2015年自贡市第一人民医院儿科收治的MPP患儿100例,根据心血管系统损伤发生情况分为A组(伴心血管系统损伤,n=23)与B组(无心血管系统损伤,n=77).比较两组患儿一般资料和实验室检查指标,小儿MPP心血管系统损伤的影响因素分析采用多因素Logistic回归分析.结果 两组患儿性别、病程、胸腔积液发生率、肺炎支原体抗体(MP-Ab)阳性率、红细胞沉降率(ESR)、白细胞计数(WBC)、清蛋白水平、血红蛋白水平、CD+4/CD+8细胞比值比较,差异无统计学意义(P>0.05);两组患儿年龄、发热时间、发热程度、大环内酯类药物使用时间、糖皮质激素使用时间、C反应蛋白(CRP)水平比较,差异有统计学意义(P<0.05).多因素Logistic回归分析结果显示,年龄〔OR=0.451,95%CI(0.279,0.729)〕、发热时间〔OR=1.919,95%CI(1.262,2.920)〕、发热程度〔OR=2.401,95%CI(1.138,5.067)〕、大环内酯类药物使用时间〔OR=0.461,95%CI(0.258,0.825)〕、糖皮质激素使用时间〔OR=0.575,95%CI(0.404,0.816)〕、CRP〔OR=1.900,95%CI(1.011,3.572)〕是小儿MPP心血管系统损伤的影响因素(P<0.05).结论 年龄、发热时间、发热程度、大环内酯类药物使用时间、糖皮质激素使用时间、CRP是小儿MPP心血管系统损伤的影响因素.%Objective To investigate the influencing factors of cardiovascular system damage in children with mycoplasma pneumoniae pneumonia.Methods A total of 100 children with mycoplasma pneumoniae pneumonia were selected in the First People′s Hospital of Zigong from 2014 to 2015,and they were divided into A group(complicated with cardiovascular system damage,n=23)and B group(did not complicate with cardiovascular system damage,n=77)according to the incidence of cardiovascular system damage.General information and laboratory examination results were compared between the two groups,and influencing factors of cardiovascular system damage in children with mycoplasma pneumoniae pneumonia were analyzed by multivariate Logistic regression analysis.Results No statistically significant differences of gender,disease course,incidence of pleural effusion,positive rate of MP-Ab,ESR,WBC,albumin,Hb,CD+4/CD+8 cell percentage was found between the two groups(P>0.05),while there were statistically significant differences of age,duration of fever,degree of fever,duration of using macrolides,duration of using glucocorticoids and CRP between the two groups(P<0.05).Multivariate Logistic regression analysis results showed that,age〔OR=0.451,95%CI(0.279,0.729)〕,duration of fever〔OR=1.919,95%CI(1.262,2.920)〕,degree of fever〔OR=2.401,95%CI(1.138,5.067)〕,duration of using macrolides〔OR=0.461,95%CI(0.258,0.825)〕,duration of using glucocorticoids〔OR=0.575,95%CI(0.404,0.816)〕and CRP〔OR=1.900,95%CI(1.011,3.572)〕were influencing factors of cardiovascular system damage in children with mycoplasma pneumoniae pneumonia(P<0.05).Conclusion Age,duration of fever,degree of fever,duration of using macrolides,duration of using glucocorticoids and CRP are influencing factors of cardiovascular system damage in children with mycoplasma pneumoniae pneumonia.
