方氏头针疗法联合艾地苯醌治疗卒中后认知障碍的临床疗效

摘要

目的 观察方氏头针疗法联合艾地苯醌治疗卒中后认知障碍的临床疗效.方法 选取2015年5月—2017年4月西安市中医医院针推康复科收治的卒中后认知障碍患者76例,采用随机数字表法分为对照组与试验组,每组38例.在常规对症治疗基础上,对照组患者给予艾地苯醌治疗,试验组患者在对照组基础上给予方氏头针疗法;两组患者均连续治疗2个月.比较两组患者临床疗效,治疗前后简易精神状态检查量表(MMSE)评分、汉密尔顿抑郁量表24项(HAMD-24)评分、Barthel指数、健康调查简表(SF-36)评分.结果 观察组患者临床疗效优于对照组(P<0.05).治疗前两组患者MMSE评分、HAMD-24评分、Barthel指数比较,差异无统计学意义(P>0.05);治疗后试验组患者MMSE评分、Barthel指数高于对照组,HAMD-24评分低于对照组(P<0.05).治疗前两组患者精力、情感职能、生理功能、躯体疼痛、精神状态评分比较,差异无统计学意义(P>0.05);治疗后试验组患者精力、情感职能、生理功能、躯体疼痛、精神状态评分高于对照组(P<0.05)).结论 方氏头针疗法联合艾地苯醌治疗卒中后认知障碍的临床疗效确切,能有效改善认知功能,减轻抑郁程度,提高日常生活活动能力及生活质量.%Objective To observe the clinical effect of Fang's scalp acupuncture therapy combined with idebenone on cognitive disorder after stroke.Methods From May 2015 to April 2017,a total of 76 stroke patients complicated with cognitive disorder were selected in the Department of Acupuncture Rehabilitation,the Traditional Chinese Medicine Hospital of Xi'an,and they were into control group and experiment group,each of 38 cases. Based on conventional symptomatic treatment,patients in control group received idebenone,while patients in experiment group received Fang's scalp acupuncture therapy based on that of control group;both groups continuously treated for 2 months. Clinical effect,MMSE score,HAMD-24 score,Barthel index and SF-36 score before and after treatment were compared between the two groups.Results Clinical effect in observation group was statistically significantly better than that in control group(P<0.05). No statistically significant differences of MMSE score,HAMD-24 score or Barthel index was found between the two groups before treatment(P>0.05);after treatment,MMSE score and Barthel index in observation group were statistically significantly higher than those in control group,while HAMD-24 score in observation group was statistically significantly lower than that in control group(P<0.05). No statistically significant differences of vigor score,emotional function score,physiological function score,somatic pain score or mental state score was found between the two groups before treatment(P>0.05);while vigor score,emotional function score,physiological function score,somatic pain score and mental state score in observation group were statistically significantly higher than those in control group(P<0.05).Conclusion The efficacy of Fang's scalp acupuncture combined with idebenone in the treatment of cognition disorders after stroke is significant. It can improve the cognitive function,reduce the degree of depression,and improve the patient's daily life ability and quality of life.
