肾癌根治术后病理分期与预后相关性分析

摘要

目的 探讨肾癌病理分期与肾癌根治术后患者生存状态及生活质量的相关性.方法 回顾性分析行肾癌根治术的70例肾癌患者, 根据肿瘤病理分期分为Ⅰ期 (A组23例)、Ⅱ期 (B组17例)、Ⅲ期 (C组16例), Ⅳ期 (D组14例) .结果 从A组到D组术后并发症及不良反应发生率逐渐上升, 且D组并发症和不良反应发生率均显著高于A组 (χ2=6.105, P=0.013;χ2=15.054, P=0.000), 但B、C、D三组相比差异无统计学意义 (P＞0.05) .通过寿命表统计分析, A组总体生存率 (术后1年、3年、5年、10年) 明显高于C、D两组.A组短期生存率 (术后1年、3年) 与B组相比差异无统计学意义 (P＞0.05), 但A组长期生存率 (术后5年、10年) 明显高于B组 (P＜0.05) .Spearman双变量相关分析发现病理分期与术后3年及10年生存率呈显著负相关 (P＜0.05), 而与术后并发症及不良反应发生率呈显著正相关 (P＜0.05) .结论 TNM分期与术后并发症和不良反应显著相关, 同时与术后生存率呈显著负相关.%Objective To investigate the correlation between renal pathological staging and survival and quality of life of patients after radical nephrectomy.Methods Recurrence Analysis Role Kidney Cancer Cruciform 70 cases Renal cancer patient.According to the pathological stage of the tumor, it was divided into stage I (23 cases in group A), stage II (17 cases in group B), stage III (16 cases in group C), and stage IV (14 cases in group D).Results From A to D group, the rate of postoperative complications and the incidence of adverse reactions were gradually increased, and D group of complications and adverse reaction rates were significantly higher group A (χ2=6.105, P=0.013;χ2=15.054, P=0.000), but B, C, D three group were no significant difference (P＞0.05).Through the statistical analysis of life table in A group, the overall survival rates (after surgery1years, 3 years, 5 years, 10 years) were significantly higher than that of C, D two group.A group the short-term survival rates (after surgery 1 years, 3 years) compared with the B group had no significant difference (P＞0.05), but the long-term survival rates of A group (after surgery 5 years and 10 years) were significantly higher than group B (P＜0.05).Spearman bivariate correlation analysis showed that pathological stage and postoperative 3 years and 10 years survival rates were negatively correlated (P＜0.05), and postoperative complications and the incidence of adverse reactions were significantly correlated (P＜0.05).Conclusion TNM staging was significantly correlated with postoperative complications and adverse reactions, and negatively correlated with postoperative survival rate.
