目的 探讨腹腔镜阴式广泛性子宫颈切除术治疗早期宫颈癌的临床效果.方法 随机选取早期宫颈癌患者60例,依据手术术式将这些患者分为腹腔镜阴式广泛性子宫颈切除术组 (腹腔镜组,n=30) 和开腹广泛性子宫颈切除术组 (开腹组,n=30) 两组,对两组患者的术中术后指标、镇痛药应用情况、术后并发症发生情况进行统计分析.结果与开腹组相比,腹腔镜组患者的术中出血量显著较少 (P ＜0.05),术后肛门排气时间、体温恢复正常时间、拔出盆腔引流时间均显著较短 (P ＜0.05);腹腔镜组患者的术后镇痛药应用率[16.7％ (5/30) ]显著低于开腹组90.0％ (27/30) (P ＜0.05).结论 在早期宫颈癌治疗中,和开腹广泛性子宫颈切除术相比,腹腔镜阴式广泛性子宫颈切除术的临床效果较好,更能促进患者术中出血量的有效减少,术后肛门排气时间、体温恢复正常时间、拔出盆腔引流时间的缩短,术后镇痛药应用率的降低.%Objective To investigate the clinical effect of laparoscopic radical vaginal hysterectomy in the treatment of early cervical cancer. Methods 60 cases of patients with early stage cervical cancer in our hospital were randomly selected, these patients were divided into laparoscopic vaginal radical trachelectomy group (laparoscopic group, n = 30) and open radical trachelectomy group (laparotomy group, n = 30) two groups according to surgical treatment methods, the intraoperation and postoperative indexes, analgesia drug applications, postoperative complications of the two groups were statistically analyzed. Results Compared with the laparotomy group, the amount of bleeding of the laparoscopic group was significantly less (P ＜ 0.05), the postoperative anal exhaust time, body temperature returned to normal time, pull out the pelvic drainage time were significantly shorter (P ＜0.05); The analgesia rate after operation of the laparoscopic group 16.7％ (5/30) was significantly lower than the laparotomy group 90.0％ (27/30) (P ＜ 0.05). Conclusion The clinical effect of laparoscopic radical vaginal hysterectomy in the treatment of early cervical cancer is better than open radical trachelectomy, it can more effectively reduce intraoperative blood loss and postoperative analgesic application, shorten postoperative anal exhaust time, recover normal time of body temperature and pull out pelvic drainage time.
