酪氨酸蛋白激酶EphA7蛋白在胃癌组织中的表达及与其预后的关系

摘要

目的 探讨EphA7蛋白表达与胃癌临床病理特征的相关性及与其预后的关系.方法 采用免疫组织化学SP法,检测83例胃癌组织和30例癌旁组织中EphA7蛋白表达水平.结果 胃癌组织中EphA7蛋白阳性表达率为81.9%(68/83),明显高于癌旁组织[23.3%(7/30)](P＜0.05).EphA7蛋白表达水平与肿瘤分化程度呈负相关性(P＜0.05);与肿瘤浸润深度、淋巴结转移、TNM分期呈正相关性(P＜0.05).EphA7蛋白表达阳性组血清CA19-9水平为(315 500±45 700)U/L,阴性组为(94 300±27 200)U/L,EphA7蛋白阳性表达组高于阴性表达组(P＜0.05).EphA7蛋白表达阴性组和阳性组平均生存期分别为48.3和22.7个月,差异有统计学意义(P＜0.05).多因素生存分析结果显示,EphA7蛋白水平、淋巴结转移和TNM分期是胃癌独立的预后因素(P＜0.05).结论 EphA7蛋白表达水平与胃癌肿瘤分化程度呈负相关性,与淋巴结转移、TNM分期和血清CA19-9水平呈正相关性.EphA7蛋白表达水平可能在胃癌的发生、发展及转移过程中发挥重要作用,可作为判断胃癌根治术后的独立预后因素之一.%Objective To explore the expression of tyrosine kinases EphA7 protein and its relationship with clinicopathological features of gastric carcinoma,and the prognosis of gastric carcinoma. Methods By S-P method of immunohistochemical staining, the level of expression of EphA7 protein was detected in paraffin-embedded specimens of 83 patients with gastric carcinoma and 30 para-carcinoma tissues. Results The positive expression rate of EphA7 protein was 81.9％( 68/83 )in paraffin-embedded specimens of 83 patients with gastric carcinoma, and difference between carcinoma tissues and para-carcinoma tissues was significant( P ＜ 0.05 ). Tumor histological differentiation had a negative correlation( P ＜ 0.05 ), while depth invasion of neoplasm,lymph node metastasis and TNM stage of gastric carcinoma had a positive correlation with the expression of EphA7 protein in gastric carcinoma( P ＜0.05 ). The serum CA19-9 levels in the positive group with expression of EphA7 protein was ( 315 500 ±45 700 )U/L and was obviously higher than that in the negative group( 94 300 ± 27 200 )U/L( P ＜ 0.05 ). The mean survival time of the patients with negative expression EphA7 protein was higher than that of the patients with positive expression( 48.3vs. 22.7 months, P ＜ 0.01 ). Multivariate survival analysis revealed expression of EphA7 protein, lymph node metastasis and TNM stage of neoplasm were significant independent prognosis factors of the patients with gastric carcinoma( P ＜ 0.05 ). Conclusion The expression of EphA7 protein in gastric carcinoma has a negative correlation with Gastric neoplasm histological differentiation, while has a positive correlation with lymph node metastasis,TNM stage of neoplasm and serum CA19-9 level concentrations. The expression of EphA7 protein maybe play an important role in occurred, developed and metastasized of neoplasm in gastric carcinoma, and used judgment as one of independent prognostic factors after surgical radical resection of gastric carcinoma.
