2014～2017年翔安区及周边县区粪常规OB结合肠镜筛查结直肠癌的检出效果评价

摘要

目的 评价2014年至2017年翔安区及周边县区粪常规OB结合肠镜筛查结直肠癌的检出效果.方法2014年1月至2017年1月间从翔安区及周边县区的社区居民随机性筛查18000例接受粪常规OB体检者纳入本研究,对粪常规OB体检阳性患者进行肠镜检查.分析受检者肠癌、腺瘤、非腺瘤性息肉检出率,不同年龄段结直肠息肉检出情况,不同年龄段结直肠癌检出情况.结果 (1)18000例体检者中肠癌49例(1.51%),腺瘤499例(17.87%),非腺瘤性息肉131例(4.69%),息肉630例(22.56%).(2)和女性患者对比,男性患者在年龄段51~60、61~70、71~74岁的结直肠息肉检出率明显更高,数据差异明显,P<0.05;其他年龄段的不同性别检出率无明显差异,P>0.05.(3)和女性患者对比,男性患者在年龄段51~60、61~70岁的结直肠癌检出率明显更高,数据差异明显,P<0.05;其他年龄段的不同性别检出率无明显差异,P>0.05.结论 结直肠息肉及结直肠癌和年龄有关,随着年龄增长而增加,且存在性别差异.超过40岁伴随肠道症状患者建议进行粪潜血(FOBT)初步筛查,阳性患者建议进行肠镜检查以便早发现早治疗.%Objective To evaluate the results of the detection of colorectal cancer in xiang'an area from 2014 to 2017. Methods 18000 patients in xiang'an district were selected.Colorectal cancer,adenoma,non-adenomatous polyps detection rate, different age of colon polyp detection,different age of colon tumor detection were analyzed.Results (1)A total of 18000 cases received physical examination,including colorectal cancer in 49 cases(1.51%),adenoma in 499 cases(17.87%),non adeno-matous polyps in 131 cases(4.69%),polyps in 630 cases(22.56%).(2)Compared with female patients,the detection rate of colonic polyps was significantly higher in men aged 51~60,61~70 and 71~74,with significant difference,P<0.05;There was no significant difference in the detection rate of different genders in other age groups,P>0.05.(3)Compared with female pa-tients,the detection rate of colorectal cancer was significantly higher in men aged of 51~60 and 61~70,with significant differ-ence in data,P<0.05;There was no significant difference in the detection rate of different genders in other age groups,P>0.05. Conclusion Colon polyps is related with colon cancer and age,and increases with age,it has gender differences.Patients over 40 with intestinal symptoms should receive FOBT initial screening.The positive patients are recommended colonoscopy screening for early detection and early treatment.
