社区慢性病患者实施医院-社区-家庭互动护理管理模式的效果观察

摘要

Objective:To discuss the effect observation of hospital-community-family mutual care management mode implemented by community patients with chronic diseases to provide guidance for the reform and healthy development of grassroots medical and health insurance system. Method:According to the existing resident health record of community health service center,selected 50 cases of patients with tumor,cerebrovascular disease,diabetes and pulmonary heart disease respectively as study objects and divided them into control group and research group respectively. Control group adopted conventional manage-ment mode,research group took hospital-community-family mutual care health management mode. Compared the medication compliance,depression and anxiety, living quality and satisfaction of two groups patients after six-month intervention. Result:The medication compliance grade of research group after intervention was higher than that of control group (P<0. 05);the depressed and anxious number was less than that of control group (P<0. 05);quality of life was better than that of control group (P<0. 05). Conclusion:The implementation of hospital-community-family mutual care management mode for community patients could release the psychological states of patients,improve mediation compliance, quality of life and satisfaction.%目的:探讨对社区慢性病患者实施医院-社区-家庭互动管理模式的效果观察,为基层医疗卫生保障系统的改革和健康发展提供指导意见.方法:根据辖区内社区卫生服务中心现有居民健康档案,选择肿瘤、脑血管病、糖尿病、肺心病患者各50例作为研究对象,并随机等分为对照组和研究组,对照组采用常规管理模式,研究组进行医院-社区-家庭互动护理健康管理模式.比较两组患者干预后服药依、抑郁、焦虑,患者的生存质量及干预6个月后患者满意度情况.结果:干预后研究组服药依从性评分高于对照组(P<0.05),抑郁、焦虑阳性人数少于对照组(P<0.05),生存质量优于对照组(P<0.05);干预6个月后研究组患者满意度优于对照组(P<0.05).结论:对社区慢性患者实施医院-社区-家庭互动管理模式可以缓解患者心理状态,提高药物服用依从性、生存质量及满意度.
