多学科团队信息化支持与冬夏令营活动的联合延续护理对1型糖尿病青少年患者的影响

摘要

Objective:To observe the application effect of multi-discipline team information support combined with winter and summer campaigns in continuing nursing of adolescent patients with type 1 diabetes.Methods: Selected 130 adolescent patients initially diagnosed of type 1 diabetes admitted in our hospital from June 2013 to July 2015 as the research target, and they were equally divided into control group and experimental group with random number table method, the patients in the control group received conventional telephone follow-up visit continuing nursing, and continuing nursing of multi-discipline team information support combined with winter and summer campaigns was employed for the patients in the experimental group on the basis of the control group, and patients in the two groups were compared in self-management efficacy and self-management behavior before and after interference, and re-hospitalization 6 months after interference.Results: After interference, the self-management efficacy and self-management behavior scores of adolescent patients with type 1 diabetes in experimental group were significantly higher than those of patients in the control group (P<0.05),the re-hospitalization due to poor blood glucose control and re-hospitalization due to diabetes complications of patients in the experimental group were significantly lower than those of patients in the control group(P<0.05).Conclusion:The interference to adolescent patients with type 1 diabetes through multi-discipline team information support combined with winter and summer campaigns in continuing nursing could significantly improve the self-management efficacy and self-management behavior of such diseases, and reduce the re-hospitalization probability due to poor blood glucose control and diabetes complications.%目的:观察多学科团队信息化支持与冬夏令营活动的联合延续护理在1型糖尿病青少年患者中的应用效果.方法:选择2013年6月~2015年7月收治于我院的1型糖尿病青少年初诊病例130例作为研究对象,以随机数字表法等分为对照组和试验组,对照组接受常规电话随访式延续护理,试验组在对照组基础上加用多学科团队信息化支持与冬夏令营活动的联合延续护理,比较两组患者干预前后自我管理效能、自我管理行为及干预后6个月内再入院情况.结果:试验组1型糖尿病青少年入选病例干预后的自我管理效能及自我管理行为评分均显著高于对照组(P<0.05),试验组因血糖控制不佳再入院率和因糖尿病并发症再入院率均显著低于对照组(P<0.05).结论:采用多学科团队信息化支持与冬夏令营活动的联合延续护理对1型糖尿病青少年患者实施干预,可显著提高该类病例的疾病自我管理效能和自我管理行为水平,降低因血糖控制不佳和糖尿病并发症所致的再入院几率.
