经皮心肺辅助循环治疗重症心肺衰竭患者的护理

摘要

Objective To explore the nursing strategy of percutaneous cardiopulmonary support system (PCPS) in patients with severe cardiorespiratory failure.Methods Of 13 patients with severe cardiorespiratory failure whose clinical data were retrospectively analyzed,5 patients with severe main coronary heart disease underwent percutaneous coronary intervention(PCI) and 8 with cardiorespiratory failure patients underwent PCPS.Results Five PCI patients completed the surgical treatments smoothly and recovered fully.Of the rest 8 cases with cardiorespiratory failure, 6 improved obviously in cardiorespiratory function,4 were cured and 2 died of multiple organ failure after cardiac badly damaged.Conclusion PCPS is an effective treatment for severe acute patients with cardiorespiratory failure.Intensive management and effective nursing play an vital role in ensuring the PCPS success.%目的 探讨急性重症心肺衰竭患者应用经皮心肺辅助循环系统(percutaneous cardiopulmonary support system,PCPS)的护理对策.方法 回顾性总结2004年12月至2010年9月应用PCPS支持的13例重症心肺衰竭患者的临床资料,其中危重冠心病严重主干病变行经皮冠状动脉介入( percutaneous coronary intervention,PCI)治疗术中行PCPS支持5例,心肺功能衰竭行床边PCPS治疗8例.结果 5例PCI患者均顺利完成手术治疗,治愈出院.8例心肺功能衰竭患者中有6例心肺功能明显好转,其中4例治愈出院;其余2例患者因心功能损害严重,死于多器官功能衰竭.结论 PCPS是治疗急性重症心肺衰竭患者的有效手段,辅助治疗期间严密的病情观察和有效的护理是提高PCPS救治成功率的重要保障.
