老年下肢深静脉血栓患者口服华法林的延续护理

摘要

目的：总结老年下肢深静脉血栓患者服用华法林抗凝治疗的延续护理.方法回顾性分析2014年1月至2015年12月在南通市第三人民医院住院的老年下肢深静脉血栓患者22例的临床资料.所有患者均实施延续护理,对患者出院后4、12周的服药依从性、血浆凝血酶原时间国际标准化比值(international normalized ratio,INR)达标率以及出血发生率进行比较.结果22例患者服用华法林治疗过程中,出院后4、12周服药依从性分别为72．7％,77．3％,血浆凝血酶原时间 INR的达标率分别为90．9％,95．5％,出血发生率分别为9．15％,4．5％.结论出院后延续护理干预可提高患者服药依从性,血浆凝血酶原时间 INR的达标率及减少并发症出血的发生,确保治疗的有效性.%Objective To conclude the continue nursing for elderly patients with lower extremity deep venous thrombosis receiving oral Warfarin.Methods Clinical data of 22 cases were analyzed retrospectively. All patients received continue nursing,the medication compliance 4 and 1 2 weeks after discharge,the target rate of international normalized ratio (INR)and incidence rate of hemorrhage were compared between two groups.Results Among 22 cases,the medication compliances in 4 and 12 weeks after discharge were 72.7%and 77.3%,respectively.The INR target rates were 90.9% and 95.5%,respectively,and the incidence rates of hemorrhage were 9.15% and 4.5%,respectively.Conclusions The continue nursing interventionafter discharge can improve the medication compliance,INR target rate and reduce the incidence rate of hemorrhage,and ensure the effectiveness of treatment.
