多巴酚丁胺负荷超声心动图检查在急性心肌梗死患者中的临床应用

摘要

目的　评价多巴酚丁胺负荷加超声心动图(DSE)检测急性心肌梗死(AMI)后存活心肌的临床价值。方法　选择38例AMI患者于发病后3-4周行DSE检查，对比观察用药前后梗死区域室壁运动得分指数(WMSI)和室壁增厚率(△T%)，识别存活心肌。结果　38例患者静息状态下104段心肌运动减弱或消失，DSE后有72段心肌恢复运动。结论　DSE是检测存活心肌准确、可靠、安全、简便的方法，具有十分重要的临床意义。%Objectives　To evaluate the clinical value of dobutamine stress echocardiography(DSE)in detecting myocardial viability in patients with acute myocardial infaction(AMI).Methods　38 patients with AMI underwent DSE during 3 or 4 weeks after AMI.The effects of dobutamine infusion of wall motion score indexes(WMSI)and thickening rate were evaluated by echocardiography to identity myocardial viability.Results　104 of 38 myocardial segments were akinesia.After dobutamine infusion,72/104 segments regained viability.Conclusion　DSE is a accuracy reliable safe simple method for detecting myocardial viability after AMI with clinical significance.
