铜绿假单胞菌的耐药特征及oprD2基因的检测

     

摘要

Objective To investigate drug resistance and the deletion of outer membrane channel protein oprD2 gene in Pseudomonas aeruginosa. Methods Agar dilution method was used to detect the minimal inhibitory concentrations(MIC) of P.aeruginosa, polymerase chain reaction(PCR) was used to amplify oprD2 gene. Results The first and the second detection rate of P.aeruginosa are ICU ward and respiratory ward, which accounted for 50.8% and 23.1%; of the 65 clinical isolates,the resistant rate to cefoperazone/sulbactam was the lowest(30.8%), the second is imipenem(38.5%), meropenem(40.0%) and amikacin(46.2%); In the 25 strains of imipenem-resistant P.aeruginosa, 19 strains were lack of outer membrane channel protein oprD2 gene, the loss rate of which is 76.0%. Conclusion The drug resistance phenomenon of P.aeruginosa is serious; the loss or reduction of outer membrane protein OprD2 may contribute to the resistance of P.aeruginosa to imipenem.%  目的了解铜绿假单胞菌临床分离菌株的耐药特征及外膜通道蛋白oprD2基因的缺失情况.方法琼脂稀释法检测最低抑菌浓度(MIC),聚合酶链反应(PCR)检测oprD2基因.结果铜绿假单胞菌检出率最高的是ICU病房和呼吸科病房,分别占50.8%和23.1%;65株铜绿假单胞菌对13种抗生素耐药率最低的是头孢哌酮/舒巴坦(30.8%),其次是亚胺培南(38.5%)、美罗培南(40.0%)和阿米卡星(46.2%);在25株耐亚胺培南铜绿假单胞菌(IRPA)中,有19株oprD2基因缺失,缺失率为76.0%.结论铜绿假单胞菌耐药性严重;外膜通道蛋白oprD2缺失可能是铜绿假单胞菌对亚胺培南耐药的重要原因之一.
