体外冲击波对兔激素性股骨头坏死血中ET-1含量及表达的影响

     

摘要

Objective To investigate the effect of extracorporeal shock wave on the change of endothelin-1 in blood plasma and endothelin-1 expression in glucocorticoid-induced femoral head necrosis rabbits. Methods Fifty-four healthy adult male Japanese rabbits, random selection 18 as control group(group A), other 36 rabbits randomly divided into model group(group B), shock wave therapy group(group C) after established glucocorticoid-induced necrosis models of femoral head, each group have 18 rabbits. Group C were start given shock wave therapy to the femoral head after the dose to the rabbit in the second week, group A group B both didn't give. Each group were randomly selected 6 rabbits take ear arterial blood after the dose to the rabbit in the 4, 6, 8weeks, at the same time use the air embolism method put rabbits to death and get femoral head. Hematoxylin-eosin staining was used to observe the histological changes of glucocorticoid-induced femoral head, enzyme-linked immuno sorbent assay was used to measure osteoprotegerin content in blood of rabbits, immunohistochemical stains observed ET-1 positive expression in glucocorticoid-induced femoral head and the expression was semi-quantitative analysised. Results Plasma ET-1 content:there was no difference between B group and C group at week 4 after the dose to the rabbit(P >0.05), but both was higher than A group(P <0.05). At week 6, 8, there was no difference between C group and A group(P >0.05), but both of them were lower than B group(P <0.05). Positive expression of ET-1 in femoral head: At week 4, 6 and 8 after the dose to the rabbit, no difference between C group and A group was observed(P >0.05), but both of them were lower expression than B group(P <0.05). Conclusion ESW can reduceET-1 expression in femoral head of glucocorticoid-induced femoral head necrosis rabbits and can Induction and strengthen osteeblast proliferation and promote new bone formation in the avascular necrosis of rabbit femoral head.%  目的观察体外冲击波疗法对兔激素性股骨头缺血坏死血浆中血管内皮素-1(endothelin-1,ET-1)的变化及表达的影响.方法健康成年雄性日本大耳兔54只,随机选取18只为正常对照组(A组),余36只造模后随机分为模型组(B组)、冲击波治疗组(C组)各18只,C组给药后第2周行兔股骨头冲击波干预,A组、B组不干预.各组于第4、6、8周随机选取6只兔耳动脉采血后处死,ELISA法测血ET-1含量.留取股骨头标本,HE染色法观察股骨头病理组织学变化,免疫组化染色观察股骨头中ET-1阳性表达,并进行半定量分析.结果4周时B组与C组血浆中ET-1含量无差别(P >0.05),但均较A组偏高(P <0.05);6周、8周时血浆中ET-1含量B组较A组、C组更高(P <0.05),A组、C组间无差别(P >0.05).结论冲击波干预兔激素性股骨头坏死后,血浆中ET-1含量减低,促进濒临坏死骨的修复和再生,防止了骨塌陷的出现.
