目的:研究慢性胃炎癌前病变(PLGC)中医标本兼治的临床疗效.方法:临床采集PLGC患者,将经本虚标实证候的100例作为治疗组,经西药维霉素治疗的100例作为对照组,两组均6个月为1疗程,疗程结束后进行临床疗效评估.结果:治疗组总疗效率为83.7％,对照组为68.2％,其差异有统计学意义(P＜0.05).5种本虚标实证候的标本兼治之间的疗效差异无统计学意义.治疗组在改善痞胀、胃痛、灼热、纳差等症状疗效显著(P＜0.01,P＜0.05)；胃镜、病理疗效的总有效率为治疗组78.3％、对照组61.4％,差异有统计学意义(P＜0.05).结论:中医标本兼治对改善PLGC中医证候、临床症状、病理状态、减轻Dys和IM程度等方面疗效均优于对照组,表明中医标本兼治能有效阻止PLGC向胃癌的方向发展.%This study was aimed to evaluate clinical therapeutic effect of precancerous lesion of gastric cancer (PLGC) with syndrome differentiation and treatment related with Xu-Shi (deficiency-excess) and Biao-Ben (branch-root). Patients with PLGC were selected and classified into the test group (n=100) treated by syndrome differentiation and treatment related with Xu-Shi (deficiency-excess) and Biao-Ben (branch-root), the control group (n=100) treated with western medicine. The treatment course of both groups was six months. Then clinical effect was evaluated. The results showed that the total clinical effective rate of the test group was 83.7%, and the control group was 68.2%. There was significant difference between two groups (P<0.05). There was no significant difference among five types of Xu-Shi (deficiency -excess) syndromes. Test group has significant effect in improving gastric distension, stomach pain, heartburn, loss of appetite (P<0.01, P<0.05). The pathological and gastroscopy effect of the total effective rate in test group and control group were 78.3% and 61.4%. It has statistical differences (P<0.05). It is concluded that syndrome differentiation and treatment related with Xu-Shi (deficiency-excess) and Biao-Ben (branch-root) has predominance in improving TCM symptom complex, clinical symptom, pathologic state and patient reported outcomes, Dys and IM degree relieving compared with the control group. It shows that TCM diagnosis and treatment is effective in the prevention of PLGC developed into gastric cancer.
