川芎嗪对脑缺血再灌注损伤保护作用的临床研究

     

摘要

objective: It is to study the protective effect of Tetramethylpyrazine on cerebral ischemia/reperfusion injury and to explore its possible mechanism. Methods 60 patients with early middle cerebral-artery occlusion ( MCA ) confirmed by cerebral angiography were randomly divided into control group ( 30 cases ) and experimental group ( 30 cases ). Besides the same general assistant treatment, all the patients of the two groups were given the right femoral artery intervention and thrombolytic therapy. Furthermore, the patients of experimental group were given intravenous infusions of Tetramethylpyrazine. Both the treatment courses were two weeks. Whether cerebral infarction occurred was confirmed and volume of the infarction was detected, neurologic impairment scores were evaluated in two weeks after operation. Results After two weeks' treatment, there was a significant difference in neurologic impairment scores between experimental group and control group ( P < 0. 05 ) , the average volume of cerebral infarction of experimental group was much smaller than that of control group ( P < 0. 05 ). There was no serious complication during the treatment of two groups. Conclusion Tetramethylpyrazine can effectively treat for cerebral ischemia/reperfusion injury, and its mechanism may be related with its antioxidation, reducing calcium over-load and inhibiting the inflammaLory response.%目的 探讨川芎嗪对脑缺血再灌注损伤的保护作用及其可能机制.方法 将60例经脑血管造影证实的早期大脑中动脉(MCA)闭塞患者随机分为实验组和对照组各30例,2组均经右侧股动脉行介入溶栓及相同的常规辅助治疗,实验组同时给予川芎嗪静滴,疗程2周.术后2周查脑CT,观察是否存在脑梗死及测量脑梗死体积,并进行神经功能缺损评分.结果 治疗2周后,实验组神经功能缺损评分与对照组相比有显著性差异(P<0.05),脑梗死体积小于对照组(P<0.05);2组在治疗过程中均未出现严重并发症.结论 川芎嗪可有效对抗脑缺血再灌注损伤,其机制可能与川芎嗪具有抗氧化、减轻钙超载、抑制炎性反应作用有关.
