4种肿瘤标志物联合检测对AFP阴性肝癌诊断的互补作用

摘要

Objective It is to observe the complementary role on combination detection of AFU , CEA, CA199 . TSGF in the diagnosis of AFP - negative hepatoma. Methods 58 palients with AFP - negative were selected, then the levels of CEA, CA199 of them were detected by ELISA assay . and the levels of AFU and TSGF were detected by quantitave biochemical analysis ; the sensitivity and specificity of four kings of tumor markers were evaluated hy ROC curve. Results The positive rates of AFU, CEA, CA199 and TSGF were 64％ , 28％ , 22％ and 66％ respectively, the positive rate of combined detection was 93％ , there were significant differences between the individual and combined detection, ( P ＜ 0. 05 ); four kinds of tumor markers checked out 12 kinds of positive associations, there were significant differences between comhined detection every association and series detection ( P ＜0. 05 ); the areas under ROC curve were all above 50％ reference line in four kinds of tumor markers, the sensitivity and specif'icity of four kings of tumor markers were satisfactorily. Conclusion Four kinds of tumor markers combined detection has a good complementary diagnostic and clinical application value in the diagnosis of AFP -negative hepatoma.%目的 探讨α-L-岩藻糖苷酶(AFU)、癌胚抗原(CEA)、糖抗原CA199、恶性肿瘤相关物质(TSGF)联合检测对甲胎蛋白(AFP)阴性肝癌诊断的互补作用.方法 选取58例AFP阴性肝癌患者,运用酶联免疫吸附技术检测CEA、CA199,生化定量分析法检测AFU、TSGF;采用ROC曲线评价4种肿瘤标志物(TM)实验的敏感性和特异性.结果 4种TM单项检测阳性率分别为AFU(64%)、CEA(28%)、CA199(22%)、TSGF(66%),联合检测阳性率为93%,联合与单项检测的阳性率相比均有显著性差异(P均<0.05);4种TM检出12种阳性组合方式,各种组合系列和联合检测的阳性率比较均有显著性差异(P均<0.05);4种TM的ROC曲线下面积都在50%参考线以上,实验的敏感性和特异性综合性能较好.结论 4种TM联合检测对AFP阴性肝癌的诊断有较好的互补作用和临床应用价值.
