康肾汤对慢性肾脏病2～3期患者微炎症状态的影响研究

摘要

目的：评价康肾汤治疗慢性肾脏病（CKD）2～3期患者的临床疗效与安全性。方法将96例慢性肾脏病2～3期患者随机分为2组，康肾汤组48例在西医常规治疗基础上同时给予康肾汤治疗，对照组48例仅给予西医常规治疗，2组均连续治疗6个月。观察2组治疗前后血肌酐（SCr）、估算肾小球滤过率（eGFR）、超敏C反应蛋白（hs－CRP）、白细胞介素－17（IL－17）、干扰素－γ（IFN－γ）、血红蛋白、血浆白蛋白及ALT的变化。结果对照组治疗后SCr、eGFR水平与治疗前比较差异均无统计学意义（P均＞0．05），康肾汤组治疗后SCr、eGFR水平较治疗前及对照组明显改善（ P均＜0．05）。2组治疗前后血红蛋白、血浆白蛋白、ALT水平比较差异均无统计学意义。治疗后康肾汤组hs－CRP水平较较治疗前明显下降（P＜0．05），而对照组较治疗前明显升高（P＜0．05），康肾汤组明显低于对照组（P＜0．05）；2组治疗后IFN－γ、IL－17水平均较治疗前明显下降（P均＜0．05），且康肾汤组水平明显低于对照组（P均＜0．05）。结论康肾汤能明显改善慢性肾脏病2～3期患者的微炎症状态及肾功能，并具有较好的安全性。%Objective It is to observe the clinical efficacy and safety of Kangshen decoction on stage 2-3 chronic kidney disease ( CKD) patients.Methods 96 cases of stage 2-3 CKD patients were randomly divided into Kangshen decoction group (n=48) treated with normal western medicine combined with Kangshen decoction and the control group (n=48) treated with western medicine alone , both groups were treated for 6 months.The changes of SCr , eGFR, hs-CRP, IL-17, IFN-γ, Hb, plasma albumin and ALT were observed before and after treatment in both groups .Results Compared with that before treatment, the differences in the levels of SCcr and eGFR in control group were not significant (P>0.05), but the levels in Kangshen decoction group were both improved obviously compared with that before treatment and in control group (P<0.05). There was no significant difference in the levels of Hb , plasma albumin and ALT in both groups before and after treatment . The level of hs-CRP was significantly decreased in Kangshen decoction group after treatment compared with that before treat -ment (P<0.05), but that in control group showed an obvious increase after treatment and was significantly higher than that before treatment and in Kangshen decoction group after treatment (P<0.05).Compared with the pre -treatment, IL-17 and IFN-γwere significantly decreased in control group and Kangshen decoction group, and the levels in Kangshen decoction group were more lower than that in control group after treatment (P<0.05).Conclusion Kangshen decoction can significantly improve micro-inflammatory state and renal function with a reliable security in stage 2-3 CKD patients .
