Objective It is to study the effect of thymopentin combined with conventional chemotherapy on respiratory function and immune function in patients with retreatment bacterial positive pulmonary tuberculosis.Methods 126 cases of patients with retreatment bacterial positive pulmonary tuberculosis were selected and randomly divided into control group and treatment group,63 cases in each group.The patients were treated with conventional chemotherapy in the control group,and with thymopentin on the basis of the control group in the treatment group,and the treatment was continued for 6 months.After treatment,the sputum negative conversion rate,focus absorption rate,narrowing and closing rate of tuberculous cavity in the 2 groups were counted;and the respiratory function and immune function between the 2 groups were compared;the adverse reactions were recorded in 2 groups.Results After treatment,the sputum negative conversion rate,focus absorption rate,narrowing and closing rate of tuberculous cavity in the treatment groups were significantly higher than those in the control group (all P ＜ 0.05);the FEV1,FVC,CD4 + and the values of FEV1/FVC,CD4 +/CD8 + were significantly heightened but CD8 + was significantly decreased in the treatment group (all P ＜ 0.05),and the improvement was significantly better than that of the control groups (all P ＜ 0.05);there was no significant difference in the serum levels of IgA,IgG and IgM of the two groups before and after treatment (all P ＞ 0.05);the incidence of adverse reactions in the treatment group was lower than that in the control group,but there was no significant difference between the 2 groups (P ＞ 0.05).Conclusion Thymopentin combined with conventional chemotherapy can significantly promote the sputum negative conversion rate in patients with retreatment of pulmonary tuberculosis,accelerate the focus absorption rate,increased the narrowing and closing rate of tuberculous cavity reduction and closure rate of pulmonary cavity,improve respiratory function,and enhance the cellular immune function of the patients.%目的 分析胸腺五肽联合常规化疗方案对复治菌阳肺结核患者呼吸功能、免疫功能的影响.方法 选取复治菌阳肺结核患者126例,随机分为对照组和治疗组各63例,对照组给予常规化疗方案治疗,治疗组在对照组治疗基础上加用胸腺五肽治疗,2组均连续治疗6个月.治疗后统计2组患者痰菌转阴率、病灶吸收率及空洞缩小、闭合率;比较2组患者呼吸功能、免疫功能相关指标;记录2组不良反应.结果 治疗后治疗组痰菌转阴率、病灶吸收率及肺空洞缩小、闭合率均显著高于对照组(P均＜0.05);治疗后治疗组FEV1、FVC、FEV1/FVC,外周血CD4+及CD4+/CD8+均显著升高而CD8+显著降低(P均＜0.05),且均较对照组显著改善(P均＜0.05);治疗后2组血清IgA、IgG、IgM水平均无显著变化(P均＞0.05);治疗组不良反应发生率较对照组低,但差异无统计学意义(P＞0.05).结论 胸腺五肽联合常规化疗方案可明显促进复治菌阳肺结核患者痰菌转阴,加快病灶吸收,提高肺空洞缩小、闭合率,改善患者呼吸功能并增强其细胞免疫功能,安全有效.
