晚期非小细胞肺癌组织中 BRCA1、RAP80的表达与化疗疗效的关系

Objective To analyze associations of gemcitabine -platinum chemotherapy effects with BRCA 1 and RAP80 expressions in Ⅲ-IV non-small cell lung cancer ( NSCLC ) .Methods The immunohistochemical method was used to detect expression of BRCA 1 and RAP80 in 62 patients with Ⅲ-IV NSCLC.Chi‐square test was used to evaluate the association of chemotherapeutic effect with BRCA 1 and RAP80 expressions .Spearman rela-tionship was conducted to evaluate the associations among expression of BRCA 1 and RAP80.Results In these 62 cases, positive expression of BRCA1 and RAP80 had no correlation with age, gender, smoking, PS score, pathology and clinical stage .The patients with negative BRCA 1 and RAP80 expression in Ⅲ-IV NSCLC were more sensitive to gemcitabine-platinum chemotherapy compared with patients with positive BRCA 1 and RAP80 expression.There were statistical differences (p<0.05);The expression levels of BRCA1 and RAP80 were positive correlated (r=0.625, p<0.01).Conclusion Positive BRCA1 and RAP80 expression are correlation with platinum resistance;The expression of BRCA1 and RAP80 were positive correlated, which showed that BRCA1 repaired DNA through RAP80 participation .%目的：研究人类乳腺癌易感基因（ BRCA1）及受体相关蛋白80（ RAP80）在晚期非小细胞肺癌（ NSCLC）中的表达及其与化疗疗效的关系。方法应用免疫组织化学（ IHC）方法检测62例经病理确诊为晚期NSCLC患者石蜡切片标本中BRCA1及RAP80蛋白表达的情况，分析其与吉西他滨联合顺铂（ GP）方案化疗疗效的关系，同时分析BRCA1与RAP80表达水平的相关性。结果 BRCA1及RAP80蛋白表达在不同年龄、性别、吸烟史、PS评分、病理类型、TNM分期组中无明显差异；BRCA1和RAP80蛋白表达阴性组GP方案化疗有效率高于蛋白表达阳性组，两组比较差异有统计学意义（p＜0．05）；BRCA1与RAP80的表达呈正相关（γ＝0．625， p＜0．01）。结论 BRCA1及RAP80蛋白表达阳性与铂类耐药有关；BRCA1与RAP80的表达呈正相关，提示BRCA1通过RAP80参与DNA修复。
