Objective To investigate the effect of Lamaze childbirth method used at different phrases during the whole birth process on the childbirth outcomes. Methods 181 primiparas taking regular childbirth check, willing to take spontaneous delivery and indicative of spontaneous delivery in our hospital from October 2008 to August 2009 were assigned to the observation group, in which the Lamaze childbirth technique training since pregnancy for 27 weeks was performed according to different phases through during childbirth process. Another 289 primiparas controlled under the same conditions in the same period was set as the control group, in which the primiparas received neither training through pregnant period nor any instructions related to it even during childbirth process. The two groups were compared in respect of the pain level and childbirth outcomes. Result The observation group was significantly superior to the control group in respect of the pain level, the first birth process, the second birth process, the rate of uterine-incision delivery, for uterine-incision delivery (for all, P < 0.05). Conclusion The Lamaze childbirth method may effectively reduce the pain level, facilitate natural childbirth, lower the rate of uterine-incision delivery and increase the spontaneous delivery rate. Consequently, it may be worth clinically popularizing.%目的 探讨全产程分阶段应用拉玛泽分娩法对产妇疼痛及分娩结局的影响.方法 选取2008年10月-2009年8月在本院定期产检,孕周28周即开始接受拉玛泽分娩技巧培训,具备自然分娩条件,且有自然分娩意愿,分娩时接受拉玛泽分娩法的初产妇181例作为观察组,选取同期具备自然分娩条件,且有自然分娩意愿,但孕期未接受拉玛泽分娩法培训,分娩时也不进行拉玛泽分娩法的初产妇289例为对照组.比较两组产妇产时疼痛程度及分娩结局.结果 两组产妇产时疼痛程度、第1产程时间、第2产程时间、剖宫产率、剖宫产指征比较,差异具有统计学意义(均P ＜ 0.01),观察组优于对照组.结论 拉玛泽分娩法能有效降低产妇产时疼痛程度,促进自然分娩,降低因个人意愿的剖宫产率,提高自然分娩率,从而提高产科质量,值得在临床推广应用.
