构建区域分级诊疗服务模式的理论探讨

摘要

Compartmentalization management and fragmental health services provision is a major challenge to achieve medical resources optimal allocation and service performance of the overall upgrade in China. Effective Classificated Treatment model is considered to be the key to crack the health care reform. This paper is committed to patients' health management,and cosider the standardized clinical pathway as a connecting link,a new model of classificated treatment of "government dominant,hierarchical collaboration,contract management,performance pay " is proposed.%我国医疗卫生服务体系条块化管理、割裂式服务的现状难以实现医疗资源的优化配置和服务绩效的整体提升。有效的分级诊疗服务模式被认为是破解这一医改难题的关键。本研究遵循“基层首诊，双向转诊，急慢分治，上下联动”的总体原则，以患者健康管理为宗旨，以标准化临床路径为连接纽带，打破医疗机构间各自为政、割裂式服务的现状，提出了构建“政府主导、分级协作、合同管理、绩效支付”的新型分级诊疗服务模式，以期为患者提供一体化、连续性的医疗服务。
