The causes of refractory glaucoma are complex, the common filtration surgery have high failure rate. The mainly reason is fibroblasts hyperplasia and scarring on filtration mouth that hindered the drainage and diffusion aqueous. Therefore, the development trend of refractory glaucoma treatment is to reduce surgery failure rate by looking for reasonable anti metabolic drugs and establishing improved operation. This paper described the concept of refractory glaucoma,pathophysiological mechanism,summarized the research progress of anti metabolic drugs commonly used, glaucoma drainage device, trabeculectomy and destruction ciliary body,the application of vitreous retinoscope operation in refractory glaucoma treatment. Looking more advanced drugs, implanting materials and surgical method.%难治性青光眼的病因复杂,常规滤过性手术失败率高,原因主要是滤过口处成纤维细胞增殖以及瘢痕形成,阻碍了房水的引流和扩散.因此,寻找合理的抗代谢药物和建立改良术式来减少手术失败是治疗难治性青光眼的发展趋势.现对难治性青光眼的概念范畴、病理生理机制,国内外报道较多的抗代谢药物、青光眼引流装置、内引流术和睫状体破坏术的研究进展及玻璃体视网膜手术在难治性青光眼治疗中的应用作简要综述,并展望更先进的药物,植入材料和手术方法.
