目的 探讨老年股骨粗隆间骨折患者髓内钉固定的优势与不足.方法 回顾性分析2005年7月至2008年12月期间在我院进行髓内钉治疗的70岁以上股骨粗隆间骨折患者43例的临床资料,并统计骨折愈合时间及术后并发症.结果 患者随访4 ～45个月,平均17个月,骨折愈合时间70 ～120 d,平均92 d,无骨不连.并发症:术中发生粗隆下骨折2例,给予改用加长型髓内钉固定,骨折愈合;深静脉血栓6例.结论 优势:操作相对容易,出血量少,相对安全稳定,适应症更宽.不足:术中操作依赖透视机,术中新发骨折概率偏高,且有可能造成固定困难,手术创伤加大、风险增加.%Objective To discuss the advantages and disadvantages of intramedullary nailing fixation in elder patients with intertrochanteric fractures. Methods Retrospective analysis of clinical data of 43 cases of patients( >70 years old )with intertrochanteric fracture during July 2005-December 2008 in our hospital re-ceiving the treatment of intramedullary nailing, the fracture healing time and postoperative complications were statistically recorded. Results Patients were followed up for 4 to 45 months with an average of 17 months, fracture healing time 70-120 d with an average of 92 d,no nonunion. Complications:subtrochanteric fractures occurred in 2 patients,giving instead elongated intramedullary nailing,fracture healing;6 cases of deep vein thrombosis. Conclusion Pros:relatively easy operation,less bleeding,relatively secure and stable,wider in-dications. Cons: operating more dependent on intraoperative fluoroscopy, higher risk of intraoperative new fracture, and may cause fixation difficulties, increased surgical trauma and surgery risk.
